Fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs. PSCO observed that the fire detection and fire alarm systems of the cargo holds smoke sample extraction system was off. All suction valves in sampling lines were disconnected rendering the system inoperable.

The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. The various tasks should be defined and assigned to qualified personnel. PSCO found the smoke extraction sampling lines for cargo holds disconnected rendering the system inoperable. There id no SMS procedures authorizing this critical fire detection system to be disabled. An internal safety audit and corrective action within three months is required.
