PSCO observed the following: excessive fuel oil leaking from the #2 stbd fuel oil tank at the crossover manifold, excessive fuel oil leaking from the #2 port fuel oil tank at main isolation tank valve, and excessive fuel oil leaks from all high pressure fuel lines & pumps on the main diesel engine.

Due to objective evidence in deficincies issued on 27 March 2022 & 28 March 2022, the vessel is not in substantial compliance with the relevant conventions calling into question the adequacy and implemntation of the vessel's SMS under the ISM Code. An additional audit, by the certificate issuing authority, is required prior to the vessel's release from detention.

Before a ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. The rescue boat outboard engine is not working properly. Cooling water is not coming out of the port, which is an indication that the engine may be overheating. Clearly the engine is not being operated and tested as per the manufacturers requirnments. 74 SOLAS III/20.2
