PSCO observed the Garbage Management Plan had no procedures/guidance on the use of the incinerator onboard.

In a ship in which oil fuel is used, the arrangements for the storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board and shall at least comply with the requirements of regulation II-2/4.2.2. PSCO observed the following: excessive fuel oil leaking from the #2 stbd fuel oil tank at the crossover manifold, excessive fuel oil leaking from the #2 port fuel oil tank at main isolation tank valve, and excessive fuel oil leaks from all high pressure fuel lines & pumps on the main diesel engine. SOLAS 74(a) 2020 cons II-2/4.2.2

Resolution: Received, reviewed and accepted class report from NKK stating the deficiency was rectified. Valves packing at the #2 stbd F/O tank, crossover manifold, and #2 port F/O tank at isolation valve replaced. F/O lines and puncher valves on cylinders replaced.

Due to objective evidence in deficincies issued on 27 March 2022 & 28 March 2022, the vessel is not in substantial compliance with the relevant conventions calling into question the adequacy and implemntation of the vessel's SMS under the ISM Code. An additional audit, by the certificate issuing authority, is required prior to the vessel's release from detention. SOLAS74(a) 2020cons IX/3.1 ISM Code 10.1

Means shall be provided whereby normal operation of propulsion machinery can be sustained or restored even though one of the essential auxiliaries becomes inoperative. Special consideration shall be given to the malfunctioning of: a generating set which serves as a main source of electrical power. PSCO identified all auxiliary engines operated on a single seawater cooling pump due to malfunction of the secondary pump. SOLAS 74(a) 2020 cons II-1.26.3

11 - Life Saving Appliances › N/A - No Subsystem › Rescue boats

Before a ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. The rescue boat outboard engine is not working properly. Cooling water is not coming out of the port, which is an indication that the engine may be overheating. Clearly the engine is not being operated and tested as per the manufacturers requirnments. 74 SOLAS III/20.2
