In considering the design of oil filtering equipment, the admin shall have regard to the specification recommended by the organization. The design of the 15 PPM bilge alarm is not in accordance with MEPC.107(49) Part I/6.2.2. There is a needle valve on the effluent pick up line before the 15 PPM bilge alarm (oil content alarm), that when closed, still allows oil water seperator to process bilge water overboard.

The crew took action to mitigate fuel oil leaks on main engine by wrapping rags around piping and did not report the issues to the company. Vessel crew was unaware of procedures for regular inspections of machinery spaces. Temporary repairs were made to the auxiliary engines without documentation or notification to the company.
