Where the station power exceeds 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a general radiotelephone operator license or higher class license. No crew member on board had a vaild radio operator license. 47 CFR 80.159(e)

A cable penetration for the closed-circuit security camera located within the aft A-60 fire boundary does not contain any properly fire-rated approved stuffing material, which needs to be installed. 46 CFR 177.100

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided to indicate a high water level. Marine Inspectors (MIs) identified bilge high level indicator lights with dead bulbs for the steering gear and engine rooms when the alarms were activated. 46 CFR 182.530(a)
