Manholes and flush scuttles in position 1 or 2 or within superstructures other than enclosed superstructures shall be closed by substantial covers capable of being made watertight. The flush watertight hatch cover for the aft port side stern void was missing a gasket.

Except as provided in paragraph (b) of this section, each vessel must have a stability letter issued by the Coast Guard before the vessel is placed into service. This letter sets forth conditions of operation. The stability letter requires that the bilges remain empty and no alterations to water-tight bulkheads. The #1 FWD port rake void had approximately 1' of water and the manhole cover between the #1 FWD port rake void and #1 FWD starboard rake void was found removed. Empty out bilge water from #1 FWD port rake void and install manhole cover.

Resolution: The manhole cover was reinstalled on the bulkhead and the compartment was pumped out. The compartment was observed by for a couple days with no signs of water ingress.
