For tankers of 20,000 tonnes deadweight and upwards, a fixed deck foam fire-extinguishing system shall be provided complying with the provisions of the Fire Safety Systems Code. The arrangements for providing foam shall be capable of delivering foam to the entire cargo tanks deck area as well as into any cargo tank the deck of which has been ruptured. PSCO observed fixed deck foam system unable to supply adequate pressure to deck foam monitors. SOLAS 74(a), 2020 cons. II-2/10.8.1.1 FSS Code (2015) 14.2.1.1

"A" class divisions are those divisions formed by bulkheads and decks which comply with the following criteria: They are so constructed as to be capable of preventing the passage of smoke and flame to the end of the one-hour standard fire test. PSCO observed multiple holes in two a-60 class doors at the engine room entrance and engine changing room door, holes were filled with fire block material after observation. Request classification society confirm the continued acceptance of repairs. 74SOLAS(14) II-2/3.2.4 50a.c.

All lifesaving appliances and arrangements shall comply with the applicable requirements of the code. Except when otherwise stated, the normal equipment of every lifeboat shall consist of: a food ration as described in paragraph 4.1.5.1.18 totalling not less than 10,000kJ for each person the lifeboat is permitted to accomodate. These rations shall be kept in airtight packaging and be stowed in airtight packaging and be stowed in watertight containers. Inspector found 03 food rations on stbd lifeboat and 13 food rations on port lifeboat to not be vacuum sealed rendering them useless.

A certificate called International Certificate of Fitness for the Carriage of Dangerous Chemicals in Bulk, should be issued after an initial or periodic survey to a chemical tanker engaged on international voyages which complies with the relevant requirements of the code. Vessel's International Certificate of Fitness states the vessel is Type 3. On the Addendum to the Interntaional Certificate of Fitness it states the vessel is Type 2. Please confirm correct ship type on both certificaates.

An immersion suit of an appropriate size complying with the requirements of section 2.3 of the code shall be provided for every person onboard the ship. Inspectors found 3 oversized immersion suits stored forward near the liferaft required by SOLAS (14) III / 31.1.4. These 3 immersion suits are not appropriate size for the crew at this time.
