The company should establish procedures to ensure that the ship is maintained in conformity and regulations. In meeting these requirements the company should ensure that any non-conformity is reported, with its possible cause, if known.
As delineated in deficiencies 4 and 5 maintenance being conducted in accordance with ships procedures[SMM Manual 1, Form V-PIR000114/21] c rew did not identify or report any non-conformities and there possible cause when conducting maintenance on weathertight doors and bulkheads including ventilation systems.
Internal ISM audit is required focusing on maintenance procedures and reporting non-conformities.

PSCO observed multiple doors on the superstructure missing handles leaving a whole that goes through the doors and can allow the entry of water.
