Each life float must be fitted with two paddles. Each paddle must be of at least 4 feet in length, lashed to the life float to which it belongs and buoyant. Life float missing 01 paddle. Provide one additional paddle. 46 CFR 180.175(d)

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: A light to indicate discharge; An audible alarm that sounds upon discharge. No audible alarm received at operating station during starboard engine fireboy override test. Operator replaced engine shutdown override system with new relay box and helm display during inspection. Shutdown audible alarm tested and working correctly. 46 CFR 181.420

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Synthetic grab line and water light line on vessel ring buoy wasted. Operator replaced all synthetic lines during inspection. 46 CFR 176.808(b)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Overhead lights in port and starboard bow compartments wasted. Both lights removed and dead end wires capped and seperated to prevent shock. 46 CFR 183.200

All fiber reinforced plastic vessels constructed with general purpose resins must be fitted with a smoke activated fire detection system of an approved type installed in accordance with 76.27 in subchapter H of this chapter in accommodation spaces and service spaces. Vessel does not a have smoke activated fire detection system for required spaces. Provide smoke activated fire detection system and install to the satisfaction of the attending marine inspector. 46 CFR 177.410(c)(3)(ii)

A port light must have an inside, hinged dead cover. Vessels dead covers damaged on port head. Provided operational hinged dead cover. 46 CFR 179.350

Every vessel must be fitted with a suitable magnetic compass designed for marine use, compass must be illuminated or restricted to daytime operation. Compass glass cracked and deteriorated from sun damage. Provide new glass for compass. 46 CFR 184.402

Life floats and buoyant apparatus must not be stowed in tiers more than 4 feet high. When stowed in tiers, the separate units must be kept apart by spacers. Insert spacers in between Life floats. 46 CFR 180.137(f)

Rails must be installed near the periphery of all decks of a vessel accessible to the passengers and crew. Aft stairs leading to sugar scoops missing rails and courses. Provide rails and courses for aft stairs. 46 CFR 177.900

The stowage locations of the personal flotation devices carried in addition to life jackets under § 180.72, must be separate from the life jackets, and such as not to be easily confused with that of the life jackets. Type III life jackets were stored with Type I life jackets. Stow Type III life jackets in separate location. 46 CFR 180.78(b)

Boundaries that separate machinery spaces from accommodation spaces, service spaces, and control spaces must be lined with noncombustible panels or insulation approved in accordance with § 164.009 in subchapter Q. Port engine room hatch missing insulation. Install insulation. 46 CFR 177.410(c)(4)

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited, the operator must secure each Type III device which prevents discharge of sewage. Acceptable methods include - (1) closing each valve and removing the handle; (2) padlocking each valve in the closed position;(3) Using a non-releasable wire tie to hold each valve in the closed position. Vessel has type III MSD system and system was not secured to prevent discharge. Secure device. 33 CFR 159.7 (c)

Electrical equipment on a vessel must be installed to protect the vessel from electrical hazards, including fire. Discovered extension cord for the juice machine. Submit one-line diagram to hard wire juice machine. 46 CFR 183.210

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Discovered small salt water leak in PEX pipe leading into the starboard head. Fix leaking pipe. 46 CFR 176.800(a)

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. Employer’s drug and alcohol policy not onboard the vessel. Place a copy onboard. 46 CFR 16.401(a)

The employer shall provide an Employee Assistance Program for all crew members that includes both education and training on the effects and consequences of drug and alcohol use. No evidence of crew training or supervisory personnel 60 minute training. Provide evidence of required training completed. 46 CFR 16.401(b)

Sail rigging and components are to be maintained in accordance with good marine practice. Vessel’s Preventative Maintenance Plan (PMP) had not been updated with recent rigging replacements. Update PMP to current status. 46 CFR 176.800

In applying the provisions of this subchapter, the OCMI may give special consideration to authorizing departures from the specific requirements when unusual circumstances or arrangements warrant such departures and an equivalent level of safety is provided. The OCMI of each marine inspection zone in which the vessel operates must approve any special consideration granted to a vessel. Vessel is constructed of general purpose resin, and does not comply with 46 CFR 177.410(b). Submit a formal request for special consideration. 46 CFR 175.550

Each vessel must be equipment with at least two operable portable battery lights. Fixed on scene. 46 CFR 183.430

The space containing propulsion machinery must be equipped with a fixed gas fire extinguishing system in compliance with 181.410 of this subpart. The vessel's port side engine room fixed fire extinguishing system discharged. The vessel is allowed to depart Hawaii and transit to Maui with the the following fire watch plan in place. While underway the port engine room needs to be inspected every half hour and have additional portable fire extinguishers readily available. The vessel must make permanent repairs to the port engine room fixed gas fire extinguishing system and conduct operations/ test the system in the presence of an attending Coast Guard Marine Inspector and prior to operating with passengers.

Bilge high level alarm visual indicator at helm for port side lazarette (Space #4 on alarm panel) not operational. Make visual indicator operational. Conduct round on space every 30 mins and log to ensure no flooding.

Port side engine room hatch missing noncombustible panel or insulation approved in accordance with 46 CFR 164.009. Provide appropriate panel or insulation and ensure it meets 46 CFR 164.009.

Starboard side engine room bilge discharge hose kinked and cracked. Replace bilge discharge hose.
