No electrical equipment shall be installed in any space where flammable vapors are liablke to collect including those on board tankers, unless the administration is satisfied that such equipment is appropriately certified for safe usae in dusts, vapors, or gasses likely encountered.
The PSCO observed imporoper wiring in the pumproom ballast treatment plant, port and starboard midshipstore vent fans, tank overfill alarms, on lighting for port accomodation ladder, and starboard side of accomodation. Additionally PSCO observed non-certified light on starboard side of accomodation facing cargo area.

The PSCO observed leaks on the cargo tank P/V valves for cargo tanks 2STBD, 2 center and 3 center
Rectify prior to cargo operations.

The venting system shall be provided with devices to prevent the passage of flame into the cargo tanks. The design testing and locating of these devices shall comply with the requirements established by the administration based on guidelines developed by the organization.
PSCO observed improperly installed flame screen on cargo tank 5 STBD causing opening in vaccum sides.

PSCO observed two of three sample pumps for fixed gas detection system broken off mounts. Pumps were ziptied in place. Zip ties were anchored to electrical wiring pulling wiring from connections.
