The safety managment system (SMS) should include procedures ensuring thhhat non-conformities, accidents and hazardous situations are reported to the company investigated and analyzed with the objective of improving safety and pollution prevention. On March 25th, 2025, the PSC team boarding the vessel at anchor observed a dark oil-like substance underneath the ODME discharge opening on the STBD hull, which contacted the water. The PSC team observed two crew members on the BOSN's chair actively cleaning the oil-like substance off the ship's hull, and no record of this notification was found onboard during the exam. Thus, the vessel failed to report a hazardous situation to the local port state authority. An external audit of the vessel's SMS is required. 74 SOLAS (20) IX/3.1; ISM 9.1

On March 25th, 2025, the PSC team boarding the vessel at anchorage observed two crew members not wearing life jackets cleaning underneath the ODME discharge opening on the STBD side hull using the BOSN's chair. ON March 27th, 2025, the PSCO's discovered the SMS mandates the wearing of life jackets while working over the side. Thus, there is a lack of congruence with the SMS concerning working over the vessel's side that ensures the safety of the crew onboard. An external audit into the vessel's SMS is required. 74 SOLAS (20) IX/3.1; ISM 1.4.2

A ship when in a port or an offshore terminal of another Party is subject to inspection by duly authorized by such Party concerning operational requirements under this Annex, where there are clear grounds for believing that the master or crew are not familiar with essential shipboard procedures relating to the prevention of pollution by oil. PSCO while conducting an offshore inspection at Corpus Christi anchorage on 25 MAR 25 observed hardened oil like substance on the ships hull that was discharged from the ODME overboard valve. PSCO re-attended vessel on 26 MAR 25 and removed the blind flange inboard of the manual isolation ODME overboard valve and found it caked with oil like substance. MARPOL I/11.1
