A main source of electrical power of sufficient capacity to supply all those services mentioned in regulation 40.1.1 shall be provided. This main source of electrical power shall consist of at least two generating sets. Vessel reported the "harbor generator" suffered fuel leak in a hard line on the high pressure side of fuel pump. Vessel has one shaft generator inoperable and one emergency generator operable. Repairs shall be made prior to operating as a towing vessel or carrying passengers/cargo.

The vessel is required a tug escort while transiting the Port of Virginia or until the vessel generator is repaired to the satisfaction of Class. The number of tugs, horsepower, and bollard pull shall be determined by the [name withheld] the vessel experiences any propulsion or steering issues while underway you are to follow Coast Guard casualty reporting requirements.

The revised NOx Technical Code 2008 shall be applied in the certification, testing, and measurement procedures for the standards set forth in this regulation. Each marine diesel engine specified in 1.2 shall be subject to the following surveys: a pre-certification survey that shall be as such to ensure that the engine, as designed and equipped, complies with the applicable NOx emission limit contained in regulation 13. If the survey confirms compliance, the Administration shall issue an Engine International Air Pollution Prevention Certificate. Per CG-543 Policy Letter 09-01, “for all US ships engaged on international voyages, verify that all engines over 130kW/175 horsepower installed on a vessel constructed on or after January 1, 2000 have a valid EIAPP certificate issued by the EPA.” EIAPP certificates on board were issued by GL. MARPOL IV/13.8 NOx Technical Code 2008 2.1.1.1, 50c

The following fire protections systems shall be kept in good order so as to ensure their required performance if a fire occurs: means of escape systems and appliances. Means shall be provided for closing all other openings to the space. Dampers shall be provided for openings in the lower portion of the space. Port automatic vent damper is inop. Repair.renew automatic fire damper in engine room.

Each hook body of a release mechanism must be marked. The plate or label must contain the information (1)-(8). Release hook for rescue boat is not labelled with USCG approval number. Label hook IAW 46CFR160.170-17.

The following emergency lighting and power loads must be arranged so that they can be energized from the final emergency power source: each steering gear feeder, if required by part 58, subpart 58.25, of this chapter. Power indicator and alarms inoperative under emergency power conditions. Repair emergency power steering circuit to give alarms and indicators at operator station.

Vessels on an Oceans or Coastwise route and 500 GT or more or on an international voyage and 500 GT or more must have a final emergency power source that meets the requirements of 46 CFR Part 112. The vessel's diesel driven emergency generator has been removed from the vessel. Install and prove proper operation of an emergency source of power IAW 46 CFR Part 112 prior to conducting an International, Coastwise, or Oceans voyage but no later than 14 days.

On U.S. inspected ships, oily water separating equipment and bilge alarms must be approved under 46 CFR 162.050 The vessel's installed oily water separator can not be proven to operate properly. Prove proper operation of OWS to satisfaction of attending Marine Inspector. Prior to any operation that requires the use of the OWS but no later than 14 days.

An emergency means of communication required by this subpart 113.30 must— (1) Be comprised of either fixed or portable equipment; and (2) Provide common talking means of two-way voice communication and calling among the navigating bridge, emergency control stations, muster stations, embarkation stations, and other strategic positions listed in 113.30-5. Vessel's fixed sound powered phones do not announce at each respective calling station. Prove proper operation of sound powered phones.

The oil content meter for the installed OWS has not been calibrated and the IOPP Certificate is due for renewal. Provide objective evidence that the oil content meter is calibrated.

At each inspection for certification and periodic inspection the Marine Inspector shall conduct such tests and inspections of the main propulsion and auxiliary machinery and of its associated equipment, including the fluid control systems, as he feels necessary to check safe operation. Harbor ship service generator turbo charger is leaking oil from turbine side. Repair ship service generator to the satisfaction of the attending Marine Inspector. 46 CFR 61.20-3(a)

Vessels rescue boat marking with name, port, and number of persons for which the boat is equipped has become worn and unclear. Each side of the recue boat bow must be marked in block capital letters and numbers with the name of the vessel. The name of the port must be marked on the stern of the vessel to meet the requirements of 46 CFR 67.123. The numbers of persons for which the boat is equipped must be clearly marked preferably on the bow, in permanent characters.

Every ship shall be provided with radio installations capable of complying with the requirements prescribed by regulation 4. Vessel's MF/HF DSC Coupler antenna inop. VHF FM800S antenna is inop causing 1 of 2 required GMDSS consoles to malfunction. Repair antennas and provide tech report by qualified servicing technician attesting to the proper operation of all radio and GMDSS equipment. 74 SOLAS 20 Amend. Chapter IV Reg 6.1

Satellite EPIRBS shall be tested at intervals not exceeding 12 months for all aspects of operational efficiency with particular emphasis on frequency stability, signal strength and coding. The EPIRB on board was not tested in accordance with 46 CFR 80.1105(k). Test the EPIRB in accordance with 46 CFR 80.1105(k) and submit proper paper work to the satisfaction of the attending marine inspector.
