Except as provided in paragraph (a)(3) of this section, no person may operate an oceangoing ship of 400 gross tons and above but less than 10,000 gross tons, excluding a ship that carries ballast water in its fuel oil tanks, unless it is fitted with approved 15 parts per million (ppm) oily-water separating equipment for the processing of oily mixtures from bilges or fuel oil tank ballast. Vessel reported OWS leak within skin of the ship. Repair leak to the satisfaction of the Marine Inspector prior to 31 OCT 2025.

[name withheld] reported a failure in the jacket water piping on one of the vessel's propulsion generators. Provide Sector San Diego the proposed repair plan and prove proper operation of the engine.

The arrangements for the ready availability of water supply shall be to the satisfaction of the Administration. While operating the emergency fire pump, MI discovered pin hole leak on the discharge side of pump flange in the bow thruster room. Rectify to the satisfaction of class. SOLAS (09) II-2/10.2.1.2

(a) In General.-A vessel engaged in commercial service that does not carry passengers, shall maintain a video surveillance system. (b) Applicability.-The requirements in this section shall apply to- (1) documented vessels with overnight accommodations for at least 10 individuals on board that are- (A) on a voyage of at least 600 miles and crosses seaward of the Boundary Line; or (B) at least 24 meters (79 feet) in overall length and required to have a load line (c) Placement of Video and Audio Surveillance Equipment.-(1) In general.-The owner of a vessel to which this section applies shall install video and audio surveillance equipment aboard the vessel not later than 2 years after enactment of the [name withheld] Coast Guard Authorization Act of 2022, or during the next scheduled drydock, whichever is later. Vessel is currently undergoing dry-dock inspections and is required to install surveillance system. 46 U.S. Code 4901

(a) On a merchant vessel of the United States the construction of which began after March 4th, 1915..(5) each crew berthing area shall be equipped with information regarding-(A) vessel owner or company policies prohibiting sexual assault and sexual harassment, retaliation, and drug and alcohol usage; and (B) procedures and resources to report crimes, including sexual assault and sexual harassment, including information-(i) on the telephone number, website address, and email address for reporting allegations of sexual assault and sexual harassment to the Coast Guard;(ii) on vessel owner or company procedures to report violations of company policy and access resources; (iii) on resources provided by outside organizations such as sexual assault hotlines and counseling;(iv) on the retention period for surveillance video recording after an incident of sexual harassment or sexual assault is reported; and (v) additional items specified in regulations issued by, and at the discretion of, the Secretary of the department in which the Coast Guard is operating. (d) In each washing space in a visible location there shall be information regarding procedures and resources to report crimes upon the vessel, including sexual assault and sexual harassment, and vessel owner or company policies prohibiting sexual assault and and sexual harassment, retaliation, and drug and alcohol usage. Vessel missing all required signage. 46 U.S. Code 11011

(a) In General.-The owner of a vessel subject to inspection under section 3301 shall- (1) ensure that such vessel is equipped with a vessel master key control system, manual or electronic, which provides controlled access to all copies of the vessel's master key of which access shall only be available to the individuals described in paragraph (2); (2) establish a list of all crew, identified by position, allowed to access and use the master key and maintain such list upon the vessel, within owner records and included in the vessel safety management system; (3) record in a log book information on all access and use of the vessel's master key, including- (A) dates and times of access; (B) the room or location accessed; and (C) the name and rank of the crew member that used the master key; and (4) make the list under paragraph (2) and the log book under paragraph (3) available upon request to any agent of the Federal Bureau of Investigation, any member of the Coast Guard, and any law enforcement officer performing official duties in the course and scope of an investigation. Vessel found without master key control policy or logs. 46 U.S. Code 3106

Materials readily rendered ineffective by heat shall not be used for fire mains and hydrants unless adequately protected. The vessel has a temporary repair using fiberglass wrap on the vessel's suction piping of the main fire pump. Vessel to provide proof of permanent repairs to the cognizant OCMI. 74 SOLAS (09) II-2/10.2.1.1

On 09FEB2024, vessel master reported the following to USCG Sector San Diego: Water ingress found in the bossing void. Upon further investigation, it was discovered that a dime-sized hole was discovered on the stern tube causing the water ingress. A temp repair to the crack/hole was made by the vessel crew to seize water ingress. Vessel operator required to conduct full assessment of stern tube and submit repair proposal to USCG Sector San Diego based on the assessment. Vessel operator required to make repairs based on approved repair proposal.

A leaking weld was identified in the bossing space at approximately the centerline and a temporary repair was installed. Provide a permanent repair plan for USCG OCMI approval. To be rectified within 90 days (12 October 2023).

Each Class 2 and Class 3 door must have power supplies, power sources, installation tests and inspection, and additional remote operating consoles in accordance with Supplemental Requirements Nos. S1 through S4 of ASTM F 1197. An electric horn or howler shall give warning at least 5 seconds, but no more than 10 seconds, prior to door movement for class 3 doors. This warning shall continue to sound until the door is closed. Class 3 WTD going to aft steering did not have horn. 46 CFR 170.270 (c)

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures other than power-operated watertight doors and those which are normally locked shall- be self closing. Doors on vessel not fully closing. SOLAS 74 (2009 Cons) II-2/4.1.1.4

Manual fire call point in gym was inoperable. Repair/replace as necessary and conduct a satisfactory test in the presence of a USCG marine inspector.

Smoke detection ceiling unit labeling missing, and EEBD's missing from it's storage in recently modified crew berthing areas (01-19-1, 01-24-1,01-28-1,01-33-1, 01-37-2,01-39-2)

The fire screen door leading from the stairwell into the AMR compartment was found to be hitting the door jam and not properly self-closing. Prove proper operation of fire screen door.

While testing SSDG emergency shutdowns, the #2 engine shutdown would only work when the engine was started in the local position. Operation of the #2 SSDG is only permitted in local start mode until deficiency is corrected.

Deck lighting in vicinity of the rescue boat is not operational. Must make necessary repairs

General alarm sign on starboard main deck in vicinity of rescue boat was painted over. Must replace sign.

Fire extinguisher at main entrance to the galley is labeled as PKP. There is currently an ABC extinguisher installed. Must mount the appropriate extinguishers in accordance with the fire control plan.

Provide correct date/time input for Oil Content Meter recording device. MARPOL 73/78 Annex I Reg 14.6 / 33CFR 155.360

Properly install or remove the following electrical equipment found: 1) lighting fixtures (x3) permanently installed behind Trawl HPU motor control center, 2) Chest freezer in steering compartment, 3) box fan in Aux Machinery space at ventilation duct. SOLAS II-1:45 / 46CFR 111.77-3 & 111.75-20(a).

#4 SSDG - identify cause and correct leak to regulator housing (cooling system). Component replaced, 26Jun2019 received report that component failed. Deficiency updated to reflect item is still open.

Remove supplies obstructing emergency escape from machinery flat.

Install proper bolt in tank level indicator for forward F/O tank (port side glass).

Means shall be provided for recharging all engine starting, radio and searchlight batteries. Charger in rescue boat found inoperable. Replace battery charger with approved type.

Not less than 12 rocket parachute flares shall be carried and stowed near the bridge. Only 03 rocket parachute flares on board. Provide proper amount.

Not less than 50% of lifebuoys shall be provided with self-igniting lights. 02 lights found expired. Replace all expired lights.

Spare fire extinguishers (or charges) shall be provided for 100% of first ten & 50% of the remaining fire extinguishers of the same type and capacity. COI requires 75 fire extinguishers on board. Fire control plan shows 46 fire extinguishers. 14 spares identified during inspection. Replace missing spare fire extinguishers.

Gyro-compass repeaters to take bearings over a 360* horizon arc shall be fitted. Port bridge wing gyro repeater found inoperable. Repair gyro repeater.

IMO: Vsl is broadcasting invalid IMO #

Install structural insulation to the edge of the door frame on the machinery space emergency escape door.

Replace defective FO service return hoses for SSDGs # 3 and #4.

Inspect and replace defective non-metallic expansion joints throughout machinery space at next yard period.

When more than 12 nautical miles from nearest land, any dishcharge of oil or oily mixtures into the sea from a ship other than oil tanker is prohibited except when the oil content is less than 15ppm and the ship has in operation OWS equipment and bilge monitor. Discovered OCM alarm is not opertion during testing of equimpment. 33CFR151.10(a)

Discovered incinerator sludge tank recorded in ORB and not listed on IOPP. Tank identies used in ORB do not match IOPP. The ORB shall be maintained IAW 33CFR151.

Make permanent repairs to pinhole leak in port seawater intake.46 CFR 56.01-1

Repair malfunctioning fire monitoring system. Currently the system is experiencing fault codes for the incinerator and deck locker compartments. Vessel is authorized to proceed on a scheduled voyage. Mitigation of hazard will be done by performing roving watches of the affected regions every 30 mins.

Provide updates to PSTS to include procedures for overspeed and reverse power testing

Make CO2 piping connections tight

Provide 03 GMDSS libeboat radios batteries.

Provide liferaft embarcation lighting from the emergency power source.

Ship's CG approved rescue boat has sustained structural damage to the hull and requires Manufacture (Mfg) approved/authorized repairs. Complete hull repairs to ships rescue boat to the satisfaction of rescue boat Mfg and attending [name withheld] repair procedures are authorized by the Mfg and approved by the attending Coast Guard Marine inspector prior to commencement of work.

Ensure permanent repair plan is submitted to the attending CGMI and repairs completed by 12/31/16.

Conduct repairs to corroded aux seawater, port main, and water maker piping. Provide hydro (1.5x MAWP) results for aux seawater system.

Provide ultrasonic testing results focusing on known high corrosion areas downstream of heat exchangers for port aux a/c seawater, port/starboard main seawater, trawl winch, anchor windlass, and crane piping.

Provide borescope inspection report of mild steel pipe section of aux a/c seawater, port main & water maker systems.

Provide technicians report attesting to proper operation of cathodic protection system following repairs prior to being granted 2016 credit dry dock.

Provide hydro (1.5x MAWP) results for all seawater systems (except port aux a/c) downstream of heat exchangers prior to being granted 2016 credit dry dock.
