Key personnel must complete training in 33CFR101.650(d)(2) by January 12, 2026. Company does not have a CYSO and all key personnel have not yet completed the training as of this inspection. 33 CFR 101.650(d)(4)

46 CFR 176.804 At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: (a) Operation of the main propulsion machinery both ahead and astern; (b) Operational test and inspection of engine control mechanisms including primary and alternate means of starting machinery; (c) Inspection of all machinery essential to the routine operation of the vessel including generators and cooling systems; Vessel was transitting Pilsbury Sound on the way to a COI inspection with no passengers on board and experienced a lower unit failure on one of two outboard engines.

On a vessel of at least (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces:(1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette;(2) A machinery space bilge, bilge well,(3) A space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck.04 high level audible and visual alarms at the operating station are inoperable. Vessel has no operational bilge audible/visual alarms. 46 CFR 182.530

A vessel must be provided with bilge pumps in accordance with Table 182.520(a). 1 fixed hand pump (10 GPM) and 1 portable hand pump (5 GPM). Fixed hand pump was inoperable due to cracked/holed suction hose. 46 CFR 182.520

The shutoff valve at the fuel tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. Port and Stbd fuel tank shutoff valaves will not remotely open/close. Stbd remote pull is missing handle. 46 CFR 182.455

A vessel of 12 meters or more in length shall be provided with a whistle. Vessel did not have a sound producing device on board. 33 CFR 83.33

Vessels operating solely within 20 NM of land must be equipped with VHF-DSC radiotelephone installation. During the VHF radio test, the power to the radio was intermittent and the radio was inoperable. 47 CFR 80.905

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Stern light was affixed with a plastic zip tie and shifted to the port quarter. 46 CFR 183.420

Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. Port and Stbd engine tach/temp/pressure gauges at the helm are not visible. Screens are scratched and sun-faded. 46 CFR 182.410

Each launching appliance and each survival craft and rescue boat on a vessel must be in good working order and ready for immediate use before the vessel leaves port and at all times when the vessel is underway. Remove all 3 lifefloats from the vessel for full USCG inspection of the material conditions. 46 CFR 185.700

A vessel must carry one lifebuoy floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Floating light was inoperable when tested. 46 CFR 180.70

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. Lifefloat floating light inoperable during testing. 46 CFR 180.175

46CFR182.720 An enclosed space containing machinery powered by gasoline a should be double hose clamped where practicable. Fuel lines between the fixed fuel piping and the outboard engines should be double hose clamped. 46CFR182.720 Flexible nonmetallic hose must be complete with factory assembled fittings. Attending MI's noted that the fuel hoses were in poor condition and need to be replaced also the double hose clamps were missing.

46CFR182.200 (b) All engines must have at least two means for stopping the engines under any operatingh conditions. Attending MI's noted the emergency stop cable was parted for remote shut down of the fuel tank. However two means of stopping remained. One from the navigation bridge and one at the valve located at the fuel tank.

EPIRB Hydrostatic Release Unit expired.

Remove tied down cover from lifefloats to make them float free and ready for immediate use.

Masthead running light inoperable.

Replace lifejacket placard.

Provide STCW II/3 and VSO training for licensed captain prior to international voyage.

Non-rigid life float has improper repairs made to the outer shell. Remove improper repairs and provide proper repairs in accordance with manufacturer's instructions. Provide properly repaired life float for reinspection by USCG marine inspectors prior to embarking on international voyage. 46 CFR 185.700(a)

Life floats are missing float-free link identification tag. Provide tag prior embarking on international voyage. 46 CFR 180.137(b) 46 CFR 160.073-20(a)

STBD forward bilge alarm is not operable and did not provide audible and visual alarm at the operating station when tested. 46 CFR 182.530(a)

STD fuel tank fuel shutoff valve leaking gasoline in the closed position. Replace valve. 46 CFR 182.455(b)(4)

Provide double clamps for fuel supply hoses to outboard motors from the fuel filters. 46 CFR 182.455(e)(3)(v)(B)

Provide marking to identify the location of the portable fire extinguisher in the forward accommodation space. 46 CFR 181.520

Provide updated drill log entries for abandon ship, MOB and fire drills. 46 CFR 185.520(f) 46 CFR 185.524(d)

The following spaces must be equipped with a fixed gas fire extinguishing system: (5) A space containing a fuel tank for gasoline or any other fuels having a flashpoint of 110 degrees F or lower. Port and Starboard hull compartments contain independent gasoline fuel tanks and do not have fixed fire extinguishing systems installed.

There must be a means of accurately determining the amount of fuel in each fuel tank; Port and starboard fuel tank level guages inoperable.

Each vessel that operates on the high seas must have on board a FCC type accepted category 1 406 MHz EPIRB installed to float free and activate. EPIRB antenna damaged and not fit for service.

Port side high water audio/visual alarm inoperable.

All around, forward and STBD running lights inoperable.

STBD fuel tank remote shutoff valve is not marked.

Re-mark life jacket stowage.

Effective November 1st, 2019, all life floats will be required to be replaced by approved inflatable liferafts or inflatable buoyant apparatuses (IBAs) to comply with IMO SLS.14/Circ. 87 of 1989 for 100% of passengers onboard. Pending the decision results from USCG HQ to your waiver request for the carriage of IBA lifesaving appliances, this requirement will otherwise be due on 01 January 2020. Additional time may be granted pending the results of the decision by USCG HQ. If disapproved, a reasonable amount of time to acquire necessary IBAs may be granted as approved by the OCMI.

Demonstrate proper operation of both port and starboard emergency fuel shutoffs. When tested during inspection, port side exterior pull cable parted and starboard side valve was not easily actuated.

Provide currently corrected charts of operating area. On board chart was last updated in 2006.

Provide proof of drug consotium enrollment and MIS report.

Replace grab lines on lifefloat.

Provide manufacturer's recomended batteries for water lights.
