A power-driven vessel underway shall exhibit:(i) A masthead light forward. MI found that masthead light inoperable. Rectify to the satisfaction of the MI. 33 CFR 83.23(a)(i)

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel did not complete annual inspection within +/- 90-day grace period and therefore is overdue for annual inspection. Conduct COI annual inspection, to the satisfaction of the MI, prior to the carriage of passengers-for-hire. 46 CFR 176.500 (b)(1)

The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant Officer in Charge, Marine Inspection, but must not be fewer than the minimum number required by table 1 to § 181.500(b) and other provisions of this section. Vessel equipped with two 5BC fire extinguishers. Replace with one 40BC and one 10BC. 46 CFR 181.500

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. Fire extinguisher bracket next to operating station was rusted/corroded. Replace to MI satisfaction. 46 CFR 181.520

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: A light to indicate discharge. Install discharge indicator light at operating station to MI satisfaction. 46 CFR 181.420 (b)(1)

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirement: Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. Provide proof of marine radio operator permit to MI. 47 CFR 80.159 (e)(1)

Each life jacket... must be marked in clearly legible block capital letters with the vessel's name. Re-label all life jackets to MI satisfaction. 46 CFR 185.604 (b)

If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Install safe shut off mechanism to the satisfaction of the MI. 46 CFR 182.455 (b)(4)

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Vessel's fire extinguisher is mounted inside battery storage compartment. Relocate fire extinguisher bracket to safe and readily accessible location. 46 CFR 185.100

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel has not completed annual COI Inspection within allotted time frame. Complete satisfactory annual COI inspection prior to the carriage of passengers. 46 CFR 176.500(b)(1)

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. If the annual inspection reveals deficiencies in your vessel's maintenance, you must make any or all repairs or improvements within the time period specified by the OCMI. Vessel has open deficiencies past the specified time period given by the OCMI. Vessel to complete all deficiencies notated from annual inspection to the satisfaction of the marine inspector prior to the carriage of passengers.

The following spaces must be equipped with a fixed gas fire extinguishing system, in compliance with § 181.410, or other fixed fire extinguishing system specifically approved by the Commandant, except as otherwise allowed by paragraph (b) of this section: A space containing a fuel tank for gasoline or any other fuel having a flash point of 43.3 °C (110 °F) or lower. Vessels pre-engineered fixed fire fighting system's bottle not charged to working pressure. Vessel to replace or recharge system to working pressure to the satisfaction of the marine inspector.

Firefighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Vessel owner was unable to provide logged entries of completed drills. Vessel owner must complete required drills or provide proper documentation to the satisfaction of the attending Marine Inspector.

A power-driven vessel underway shall exhibit sidelights and a stern light. Vessel was unable to produce the stern light during the inspection. Vessel owner must repair or replace the stern light to the satisfaction the attending Marine Inspector.

A vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Vessel’s charts were out of date and must be replaced.

Each officer employed on any vessel must have his or her license or merchant mariner credential onboard and available for examination at all times when the vessel is operating. Operator did not have license available for the inspection. Operator must show proof of credential to the satisfaction of the marine inspector.

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions; (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; (c) Minimize accidental personnel contact with energized parts; and (d) Prevent electrical ignition of flammable vapors. Vessel had multiple dead end exposed wiring under the helm creating a hazard for personnel on the vessel. Vessel owner must properly cap those wires or remove them entirely to the satisfaction of the attending Marine Inspector.

Each survival craft must be secured to the vessel by a painter with a float-free link. Starboard side IBA’s painter not was not attached to the float-free link. Operator secured the painter line to the float-free link to the satisfaction of the marine inspector.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Vessel’s Certificate of Documentation is expired. Vessel is authorized 7-day extension to operate and renew COD. 46 CFR 67.313(a)

Electrical equipment on a vessel must be installed and maintained to: potect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in the electrical equipment, and electrical shock. Two dead end wires were located under the center console. 46 CFR 183.200 (b)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the FCC, as set forth in 47 CFR part 80. Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator. Radio operator permit was not available for inspection. 46 CFR 184.502 47 CFR 80.159(e)

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. Hydrostatic release for IBA’s are expired. 46 CFR 185.740(b)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed. Vessel reported engine failure. Operator to provide the cause and provide adequate repairs prior to carriage of passengers.

"An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. It normally includes inspection and testing of the equipment." "For fixed-gas fire extinguishing systems, the owner must provide satisfactory evidence of the required servicing to the marine inspector." Fixed system had no servicing records. Operator was unaware of any servicing done. 46 CFR 176.404(a), 46 CFR 176.810 (2)

"The frame or support of each semi-portable fire extinguisher must be welded or otherwise permanently attached to a bulkhead or deck." Fire extinguisher not mounted. 46 CFR 181.500

"The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions." Missing emergency instructions. 46 CFR 185.510

"Each ship with 6+ passengers for hire with a radiotelephone station must carry a radio operator permit." Radio station authorization certificate expired. Renew in the 60 days. 47 CFR 80.159

"Machinery must be suitable in type and design for propulsion requirements." Lower unit on outboard engine missing bolt. 46 CFR 182.200

THE MASTER AND CREW OF A VESSEL WILL BE FAMILAR WITH THE CONTENT OF AND HAVE MOUNTED AT THE OPERATING STATION, EMERGENCY INSTRUCTIONS CONTAINING THE ACTIONS TO BE TAKEN IN THE EVENT OF FIRE, HEAVY WEATHER, OR MAN OVERBOARD CONDITIONS. MI FOUND THAT THE VESSEL WAS MISSING EMERGENCY INSTRUCTIONS ONBOARD. 46 CFR 185.510(a)

A VESSEL ON AN LBS ROUTE MUST CARRY THREE HAND RED FLARE DISTRESS SIGNALS AND THREE HAND ORANGE SMOKE DISTRESS SIGNALS APPROVED IN ACCORDANCE WITH §160.037 IN SUBCHAPTER Q OF THIS CHAPTER, OR OTHER STANDARD SPECIFIED BY THE COMMANDANT. MI NOTED THAT VESSEL WAS MISSING THE REQUIRED NUMBER OF HAND ORANGE SMOKE DISTRESS SIGNALS. 46 CFR 180.68(a)

Portable extinguishers must be inspected and maintained in accordance with NFPA 10. The annual service for the vessel's 6A:40BC fire extinguisher has not been completed. Complete annual servicing of portable fire extinguisher IAW NFPA 10. 46CFR176.810(b)(1)

A qualified servicing facility must conduct annual service for fixed-gas fire extinguishing systems. The annual servicing for the vessel's pre engineered fixed fire extinguishing system has not been completed. Complete annual servicing of the pre engineered fixed fire extinguishing system IAW 176.810. 46CFR176.810(b)(2)

Any vessel of at least 5 net tons which engaged in coastwise trade must have a Certificate of Documentation. The vessel's COD is expired. Provide proof of valid COD to the satisfaction of the MI. 46CFR67.7

The inspection and maintenance of the onboard survival craft shall be logged for review by the Coast Guard. The vessel's lifesaving inspection log was not up to date. The operator updated the log to the satisfaction of the MI. 46CFR185.516

Fixed Gas Fire Extinguishing Systems must provide satisfactory evidence of the required servicing to the marine inspector. Pre-Engineered fixed gas fire extinguishing system maintenance was not completed.

Owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector of portable fire extinguishers. Operator failed to maintain proper records of maintenance. Complete servicing of B-I extinguisher by qualified servicing facility.

Any vessel of at least 5 net tons that engages in coastise trade must have a valid Certificate of Documentation. Certificate of Documentation expired February 2017.

All machinery and equipment must be maintained to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Outboard Engine fuel hose is cracked.

An operation and maintenance manual for the flammable vapor detection system must be kept on board. Owner did not have the operation manual on board and did not know how to adequately test the vapor detector.

Each boat that has a powered ventilation system shall have a label displaying the information shown in Figure 3, located in plain view of the operator and as close as practicable to each ignition switch. Information for blower not displayed at operating station. ABYC H-02 c.2.g

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Vessel only equipped with one operable portable battery light.

Portable watertight containers for distress flares and smoke signals shall be of a bright color and containers shall be clearly marked in legible contrasting letters at least 12.7 mm high: “DISTRESS SIGNALS.” Watertight container improperly labeled.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: 1. Charts of large enough scale to make safe navigation possible, 2. U.S. Coast Pilot or similar publication, 3. Coast Guard Light List. Extracts from the publications may be provided instead of the complete publication. Charts onboard not up-to-date. Copies of the U.S. Coast Pilot and Coast Guard Light List missing.

Provide missing items from the DAPI audit; 1. company policy, 2. maintenance of records, 3. posting of hotline numbers.

Rewrite emergency procedures specific to the vessel.
