The safety management system shall be maintained in accordance with the provisions of the International Safety Management Code. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations, and with any additional requirements which may be established by the company.

The water spray system should be capable of covering all areas mentioned in 11.3.1 with uniformly distributed water-spray of at least 10 L/m squared per minute for horizontal projected surfaces and 4 L/m squared per minute for vertical surfaces. PSCO observed no less than 12 water spray nozzles clogged or spraying inadequately at various protected cargo areas

Food rations shall be kept in airtight packaging. PSCO identified 12 food rations located in the lifeboats that were not airtight. Insufficient spares on board

All gasketed pipes, joints and hose connections should be electronically bonded; 4 cargo lines at manifold had missing or improperly secured bonding straps. One line was actively being used for loading cargo

Fire doors in main vertical zone bulkheads, galley boundaries, and stairway enclosures other than power-operated watertight doors and those which are normaly locked shall satisfy the following requirements: The doors shall be self-closing and be capable of closing with an angle of inclination of up to 3.5 degrees opposing closure. All stairway enclosures above deck A were observed to not be self-closing.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed a rupture on the deck water spray pipeline above the #1 cargo tank dome approximately the size of a quarter. Port state control officers also observed a hole on the deck water spray piping covering the house causing a deluge of water to pour on to the deck below. The hole on the house piping appeared to be there prior to the inspection as it was severely corroded with rust stains approximately 6 inches around the hole.

Plans and booklets shall be kept up to date; any alterations thereto shall be recorded as soon as practicable. PSCO observed that the fire control plan does not match what was physically observed regarding smoke detectors and IMO symbols for doors. Symbols located on the fire control plan were not in accordance with IMO Resolution A.952(23)

The electrical installations shall be such that the safety of passengers, crew and ship from electrical hazard will be ensured. PSCO observed water in the housing of a deck light adjacent to the cargo deck.

PSCO’s noted the above deficiencies and questions the implementation of the vessel’s safety management system. Due to the noted deficiencies, the vessel is required to receive an audit within 90 days.

Pressure relief valves should be set and sealed by a competent authority acceptable to the administration. Five of eight saftey relief valves were missing marking tags that correspond to the testing certificates, In addition, the number one starboard seal was broken. Rectify the broken seal prior to loading cargo in the number one cargo tank.

A water spray system for cooling, fire prevention and crew protection should be installed to cover exposed cargo tank domes. After one and a half hours of maintenance and testing, crew discovered a blockage in a pipe elbow causing a restriction to cargo tank number one.

1840 - Intrinsically safe wiring for high level and overfill sensors on all cargo tanks are wasted at connection points and would not prevent passing of flammable gas into the sensors. 46 CFR 154.1010(b) 10c
