The master, owner, operator, agent, or person in charge of a vessel equipped with ballast tanks that operates in the water of the United States must employ one of the authorized ballast water management methods listed in 33 CFR 151.2025(a). Vessel's ballast water management system is inoperable. Vessel has been granted an extraordinary circumstance to employ the ballast water management method listed in 33 CFR 151.2025(a)(3) to conduct cargo operations while in the Sector New Orleans COTP Zone. Oil Discharge Montoring and Control System shall be fitted with a recording device that shall provide a continuous record that shall be identifiable as to time and date. ODME recording device produced records with discrepant date during last test and operation due to undetermined cause.

Each liferaft shall be stowed with a float free arrangement. Hydrostatic releases on liferafts were stored without painter passing through weaklink.

Fire hoses shall be of non-perishable material. Fire hose had excessive leak during fire drill.

Doors required to be self-closing and not fitted with holdback hooks. Self-closing A-class fire door leading to bridge had peice of wood to kee propped open.
