The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations. In the engine room below the main engine a-frame a leak was identified with accumulation of oil and soaked rags. Chief engineer failed to notify captain of leak or accumulated oil and rags.
