1420 - Propulsion & aux. Machinery - Means shall be provided to control leaks of flammable liquids. Currently, the #2 Generator is on line, there is an active leak with an accumulation of fuel near the fuel filters. Fuel line lagging near the fuel filters was saturated, requiring replacement. Number 1 & 3 generators while not running were found with fuel leaks & #3 had a half inch of fuel pooled in the fuel filter drip tray.

The salt water cooling system was found to be modified by the ship's crew without proper documentation from the ship's classification society.

0735 - Personal equipment - fire fighting - On all ships, within the machinery spaces, emergency escape breathing devices (EEBD) shall be situated ready for use at easily visible places, which can be reached quickly and easily at any time in the event of fire. The EEBD stowage box was found latched and closed with a welding rod and unable to be opened freely. The crew utilized mechanical means to remove the welded rod.
