Whenever a accident occurs to the ship or a defect is discovered, either of which effect the safety of the ship, the master or owner of the ship shall report at the earliest opportunity to the administration, the nominated surveyor or recognized organization responsible for issuing the relevant certificate, who shall cause investigations to be initiated to determine weather a survey as required by regulation 7, 8, 9, 10 is necessary.
While investigating the failure of the 5S cargo tank unreported previous repairs were identified to the 10P and 10s cargo/slop tanks. Crew logged repairs to the 10p and 10S cargo tanks in the hot work log on board the ship but no reports could be found to class or flag at this time. Cargo tanks are identified as a critical item.
A report by class is required to ascertain the safety of the tanks and repairs that were made.

Objective evidence discovered in an expanded ISM exam following a reportable casualty reveled the following major nonconformity; the vessel failed to fully implement the requirements of the SMS as evident by deficiency #01. An external audit is required.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Serious deformation of structural members were noted in way of the 5S cargo tank.

All life saving appliances shall be in working order and ready for immediate use. Port side life boat self contained air support system air supply gauge leaked and did not give proper pressure reading. Crew renewed needle valve connected to the pressure gauge and a proper pressure was verified by PSCO's on board. 74SOLAS(20 CONS) III/20.2
