In addition to the requirements contained elsewhere in the present regulations, ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of a classification society which is recognized by the Administration in accordance with he provisions of regulation XI-1/1, or with applicable national standards of the Administration which provide an equivalent level of safety. While underway from Busan, KR to Guam, vessel experienced a reduction in propulsion due to the failure of the saltwater cooling pipe connection to the main engine jacket water cooler. Temporary repairs were made by crew and the vessel continued the voyage. Affect permanent repairs to the satisfaction of class and USCG.

A water seal, if fitted, shall be capable of being supplied by two separate pumps, each of which shall be capable of maintaining an adequate supply at all
times. The audible and visual alarm on the low level of water in the water seal shall operate at all times.
The current configuration on the vessel has been modified by use of a fire hose that is being manually controlled to allow water into the deck seal. Prior to the discharge or loading of cargo that requires inert gas, restore the IGS to it's original configuration.

Immediately after the addressing of resultant safety concerns, the owner, agent, master, operator or person in charge, shall notify the USCG whenever a vessel is involved in a marine casualty consisting of an occurrence materially and adversely affecting the vessels seaworthiness or fitness for service.
Definitions:
The term marine casualty applies to events caused by or involving a vessel and includes but is not limited to the following: any occurrence that results in any failure or occurrences, regardless of cause which impair any aspect of a vessels operation components or CARGO.
The attending Marine Inspectors noted that the inert gas systems suffered two failures, neither or which was reported to the USCG as required 46 CFR 4.05. Both vessel and company shall review established SMS procedures and ensure that future incidents involving reportable casualties to the USCG are included in the established ISM/SMS policy.

On 30Jan2023, it was discovered that the Cargo Ship Safety Radio Certificate (CSSRC) expired 10Dec2022. Upon review of SMS Document ID 6873 (Rev 3), paragraph 4 of the “Vessel Documents and Certificates – General” section indicates both the master and Program Manager will ensure inspections and surveys are completed as required. Section 1(a) states, “Where the document originates on the vessel, the Master will update the document dates in DMS.” The master is unable to update certificate records in the “Document Management System” (NS5 where SMS is located). The master follows the process identified in Section 2(a) “Program Manager or Marine Safety and Standards (MSS) Department Responsibilities” by emailing documents to MSS for entry into DMS.
It was noted during USCG attendance many moving parts may have contributed to the lapse of the CSSRC: upcoming maintenance period, authorization from FCC for ABS to issue CSSRC, vessel’s upcoming name change, and company's change of address.

The index numbers for the decommissioned incinerator did not match. The laundry and incinerator had the index numbers switched in the table of contents.
