Other oil-level gauges may be used in place of sounding pipes subject to the following conditions: in cargo ships, the failure of such gauges or overfilling of the tank shall not permit release of fuel into the space. The use of cylindrical gauge glasses is prohibited. The Administration may permit the use of oil-level gauges with flat glasses and self-closing valves between the gauges and fuel tanks. Observed self-closing valves on every single fuel tank's sight gauge were secured in depressed position with safety wire. The Comapnay should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements the company should ensure that: inspections are held at appropriate intervals, and any non-conformity is reported, with its possible cause, if known. Safety Manual Section 2.1 "General Duties of Employees and Safe Working Practices." "Not intentionally or recklessly interfere with or misuse anything provided to preserve safety and pollution prevertion on board the ship." Safety Manual Section 2.4.2 "Electrical Equipment." "When working on electrical equipment, it is essential that it is adequately isolated." "This should incluede the removal of breakers and fuses, warning notices should be posted at these positions to ensure that power is not restored whilst work is being carried out." As deliniated in deficiences 1-2, objective evidence attests to the vessel's crew failing to effectively impliment basic SMS procedures related to safe working practices, presenting a significant safety risk to the crew and fire risk to the engine room space. This is evident by the crew's systemic use of safety wire to keep every F/O tank sight glass self-closing valve depressed open. Further, the crew does not follow any tag-out procedures despite applying tape to multiple main switch board circuit breakers, potentialy forcing circuits to remain in the open position.
