11112-Each survival craft shall be stowed in a state of continuous readiness so that two crew members can carry out preparations for embarking arrangements in less than 5 mins. PSCO observed that the vessel crew when preparing the forward life raft for embarkation did not have shackles required to attached the embarkation ladder. A crew member located the shackles and had the ladder ready for embarkation in seven minutes. 74 SOLAS (20) III/13.1.3

10101-A pilot ladder shall be certified by the manufacture as complying with the regulations or with an international standard acceptable to the organization. Each ladder shall be subject to a ladder and step attachment stregt test at no more than 30 month intervals . PSCO observed the starboard side pilot ladder was manufactured in November 2020 and did not have a 30 month strenght test. 74 SOLAS (20) V/23.2.3 ISO 799-1

At least one unit shall be located that it is easily accessible to a responsible member of the crew at all times when at sea or in port. One indicating unit shall be located on navigation bridge. PSCO observed that the indicating unit on the bridge was not operating.
