On two separate occasions, MI hazardous gas meters alarmed displaying over 50 parts per million of carbon monoxide(CO) while onboard. Vessel is required to carry onboard a serviceable CO meter that alerts the master to unsafe CO levels. Install CO meter within one week. 46 CFR 176.830 (a)

If any required fixed-gas fire extinguishing systems have not been properly maintained, a qualified servicing facility may be required to perform the required inspection, maintenance procedures, and hydrostatic pressure tests. The fire suppression system protecting the forward fuel tank space was severely deteriorated, and servicing pins for both fire suppression systems were in place, preventing proper operation of the system. A qualified servicing facility must conduct maintenance of the fire suppression system and conduct satisfactory tests in the presence of a qualified marine inspector prior to carriage of passengers.
