A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Vessel is required to be inspected by U.S.C.G MI prior to the carriage of passengers. 46 CFR 176.1009(a)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection. Vessels Port engine is OOC. Repair within one week. 46 CFR 177.100

Up-to-date charts and nautical publications of large enough scale to make safe navigation possible as appropriate for the intended voyage, must be carried onboard vessels. MI found no local charts for intended voyages. Rectify within 7 days. 46 CFR 184.420 (a)(1)

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Automatic operation did not illuminate light. Rectify within 7 days. 46 CFR 182.530(c)

A vessel must comply with the applicable requirements for any radio including the requirements for a station license to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel did not have the FCC Station License onboard. Rectify within 7 days.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. On two separate occasions, MI hazardous gas meters alarmed displaying over 50 parts per million of carbon monoxide(CO) while onboard. Vessel is required to carry onboard a serviceable CO meter that alerts the master to unsafe CO levels. Install CO meter within one week. 46 CFR 176.830 (a)

If any required fixed-gas fire extinguishing systems have not been properly maintained, a qualified servicing facility may be required to perform the required inspection, maintenance procedures, and hydrostatic pressure tests. The fire suppression system protecting the forward fuel tank space was severely deteriorated, and servicing pins for both fire suppression systems were in place, preventing proper operation of the system. A qualified servicing facility must conduct maintenance of the fire suppression system and conduct satisfactory tests in the presence of a qualified marine inspector prior to carriage of passengers.

Vessel needs to update VHF radio MMSI with what;s on the current FCC station Authorization certificate.

Vessel missed it's required annual inspection (past the 90 day window).

Hull exam expired, complete successful inspection.

FWD submersible bilge pump must be permanently mounted.

Life Buoy grab line parted.

Secondary generator fuel pump booster not supported.

Generator exhaust piping not insulated.

Stability letter must be posted at operating station.

COI must be displayed for observation by passengers.

Marine Radio Operator permit required for vessels carrying at least six passengers.

Provide VHF Radio with programmed MMSI number and link to GPS.

Midship Bilge Alarm (Audible/Visual) non-operable. Repair or replace.

Master needs supervisory DAPI training. 46CFR16.401

Must provide crewmember. 46CFR15.501(b)

Crewmember must be enrolled in DAPI program. 46CFR16.210

Logs need to be updated (drills/radio).

Master needs marine operator permit. 47CFR80

Vessel did not complete required drydock, drydock was scheduled for 12JAN17. Vessel is not authorized to operate until the completion of a credit dry dock exam to the satisfaction of the attending marine inspector.

Vessel is required to carry up to date charts that are appropriate for the intended voyage. Vessel did not have any charts on board.

Station documents must be readily available to the licensed operator on duty during the hours of service of the station and to authorized Commission employees upon request. Vessel is required to have FCC Safety Certificate on-board prior to operating vessel.

Vessel is required to have alcohol testing devices readily available on-board. The alcohol testing strips on-board the vessel expired November 2015.

Vessel is required to carry a first aid kit approved under series 160.041 or a kit with equivilent equipmenet and instructions. Vessel's first aid kit had expired aspirin and burn gel.

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make available to Coast Guard officials on request. Vessel could not provide any documentation of pre-employment, random, periodic, etc. drug testing or annual summary/audit provided by consortium.

Each floating water light on a lifebuoy must be attached to a lanyard with a corrosion resistant clip. Vessel's water light was improperly configured and the lanyard was wasted. Vessel restricted to daytime operations until deficiency is resolved.

Vessel is required to have current magnetic compass deviation table or graph or compass comparison record for the steering compass in the wheelhouse.

Vessel failed to conduct third annual re-inspection of COI and was outside the window of the fourth annual re-inspection.
