Vessel's auxiliary engine fuel shut-off cable is run through a junction box which is rattling and improperly secured. The current installation is not original. Pressure vessels shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any dangers to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. Vessel's R-404A refrigerant cylinders are stowed in the engine room, which can result in serious safety hazards if a leak occurs or the pressure cessels are exposed to high heat. SOLAS 74(2004) CH II-1/Reg 26.1 Action 16ac

Where any alterations are made to a ship as to materially affect the stability information supplied to the master, amended stability information shall be provided. Stability booklet and loading manuals do not reflect any changes or updates from Class or Administration after BWTS install. SOLAS 74(2004)CH II-1/Reg5.4 Action 50ac

A cargo ship carrying grain shall comply with the requirements of the international grain code. Vessel did not provide evidence of a Document of Authorization to carry grain. SOLAS 74(2004)CH VI/REG9.1 Action 50ac

Each ship subject to regulation 6.1 shall maintain a list of equipment containing ozone depleting substances. Vessel uses R-404A and R-407A refrigerants on board for some of the vessels equipment. Vessel IAPP Supplement form did not include a list of the hydro-chloroflourocarbons on board. MARPOL Annex VI/REG12.5 Action 50ac

Master, owner, operator, agent, or person in charge of vessel subject to this subpart must properly maintain the BWMS in accordance with all manufacturer specifications. PSCO observed active CIP liquid low level alarm.

02105 - The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. The packing gland on the rudder stock for the steering gear is allowing water ingress into the steering gear room. 74 SOLAS (14) I/11(A)

15109 - The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. PSCOs requested to see the Safety Management System checklists and reporting procedures for the steering gear and checklists for the lifebuoys and fire stations. The checklist for the steering gear show that the engineers should check for water around the rudder stock and that the rudder gland should be checked and greased daily. Additionally, the Chief Engineer shall report any defects to the master which was not completed. Fire station checklists marked all hose nozzles as "good" although the correct nozzles were missing in multiple locations. The lifebuoy inspection checklist marked all lifebuoys as "good" although they had deteriorated retro-refletive tape. Due to the objective evidence in the above deficiencies, the vessel is not in substantial compliance with relevant conventions. The Captain of the Port questions the adequacy and overall implementation of the vessel's SMS under the ISM Code. An external audit is required to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the audit report to USCG when completed. 74 SOLAS (14) IX/3.1 ISM Code 10.1

07110 – Nozzles shall be of an approved dual-purpose type (i.e. spray / jet type). Multiple fire hose boxes contained nozzles that were not dual-purpose. 74 SOLAS (04) II-2/10.2.3.3.4

07105 – Doors required to be self-closing shall not be fitted with hold back hooks. Two self-closing “A” class fire doors in the galley were fitted with hold back hooks. One self-closing “A” class fire door in the stairway was tied open. 74 SOLAS (04) II-2/9.4.2.2

07103 – “A” class divisions are those divisions formed by bulkheads that are isolated with an approved non-combustible materials. PSCO observed a portion of insulation missing from the “A” class fire boundary in the emergency diesel generator room. 74 SOLAS (04) II-2/3.2.3

07126 – Means shall be provided to control leaks of flammable liquids. PSCO observed leaks on the HFO and Lube Oil purifiers as well as the filters and pumps for the purifiers. Additionally, there was a shopvac leaking oil onto the deck of the purifier room. 74 SOLAS (04) II-2/4.1.1

11117 – All lifesaving appliances shall be fitted with retro-reflective material where it will assist in detection. PSCO observed deteriorated retro-reflective tape on lifeboat and several buoys rendering it ineffective. 74 SOLAS (14) III/34 LSA 1.2.2.7

11199 – Stowage locations for the life-saving equipment shall be marked with symbols indicating if more than one device is stowed in that location, the number of devices shall also be indicated. PSCO observed the lifejackets and immersion suits in the bosun stores missing the number of devices in that location. 74 SOLAS (14) III/20.10

09298 – Guard rails fitted on superstructures and freeboard decks shall have at least 3 courses where operationally necessary. Chains fitted between 2 fixed stanchions are acceptable in lieu of guard rails. PSCO observed one chain in place, while 2 others were missing at starboard rescue boat embarkation. ICLL Reg 25 (3)(A) + (D)

14503 – Every ship of 100 gross tons and above shall carry a garbage management plan which the crew shall follow. This plan shall provide written procedures for storing garbage onboard. PSCO observed no instruction in garbage management plan for storage of garbage onboard. MARPOL V/10.2

99101 – When cylinders of Class 2 (compressed) gas materials being transported by vessel, the cylinders must be dunnaged 10 cm off any metal deck. PSCO observed oxygen and acetylene tanks stored directly on metal deck of the vessel. 49 CFR 176.200(c)

Machinery shall be adequate for the service for which it is intended, vessel reported number 4 cylinder fuel pump inoperable. (74SOLAS(04)II-1/26.1)

The company and the ship shall comply with the requirement of the International Safety Management Code. The ship was unable to produce a shipboard maintenance plan required by the Administration.

Maintenance, Testing and inspection shall be carried out based on guidelines developed by the Organization (MSC.1/Circ.1432). Found that the fixed fire detection heat detection system was not being tested as required by the organization. Requesting an audit of the Firefighting SMS procedures.

13101 - Propulsion Main Engine - Ships shall be designed,constructed and maintained in compliance with the mechanical requirements of a classification society. An oil pressure monitoring line burst on the main engine, causing a loss of propulsion. 74 SOLAS 04 II-1/3-1

Each emergency generating set arranged to be automatically started shall be equiped with starting devices approved by the administration. A second source of energy shall be provided for an additional three starts within 30 min. during starting sequence #1 battery exploded inside the battery box emergency gererator could not be started by two means.

2430While ships are operating within an ECA, the suphur content of F/O used shall not exceed .1%m/m after 1JAN15. The PSCO has clear grounds to believe the ship was operating on non-compliant F/O within NA-ECA control area upon its arrival to COTP Baltimore 4APR16. 0615- The rescue boat was not ready for immediate use, the outboard motor was removed from the transom and was not stored in the vicinity of the rescue boat. 74SOLAS14 III/20.2 Code: 30abc

0610- The free fall lifeboat was not ready for immediate use. Life boat equipment was kept in six crew seats and not securely stowed which posed a hazard to the crew if the boat was launched. In addition, there were not enough water provisions for each person the life boat is permitted to accommodate. 74SOLAS14 III/20.2 LSA Code 4.4.8 Code: 30abc

0945- Crew could not demonstrate operation of any of the ships external emergency lighting. 74SOLAS14 III/11.4 Code: 30abc

0715- The smoke detector in the crew changing room was covered with a piece of plastic and unable to detect smoke in the space. 74SOLAS14 II-2/14.1.1 Code: 17ac

0163- The sulphur content of fuels delivered to a ship specifically for combustion purposes on board that ship shall be documented by the supplier. The ship could not provide sulphur content of low sulphur fuel on board. MARPOL Annex VI 18.5 Code: 17ac

0720- The fire fighters outfits shall be kept ready for use and easily accessible location that is clearly marked and in widely separated positions. A training SCBA bottle was stored with the fire fighting outfit. 74SOLAS14 II-2/10.3.1 Code: 17ac

1735- PSCO observed bilge and sludge piping connections that if used would allow for overboard discharge of sludge. The C/E could not provide any approved drawings with this piping system. 74SOLAS14 I/11(a) Code: 17ac

1410- PSCO observed oil leaking from crankcase explosion covers. PSCO also observed an excessive amount of oil was leaking from all oil mist detectors and being collected in a bucket. 74SOLAS14 I/11(a) Code: 17ac

0610- The number of persons for which the lifeboats is approved was not permanently and clearly marked on the outside of the free fall life boat. The approval placard on the inside showed a rating for 20 persons and the SOLAS Safety Equipment Certificate form E rated the life boat for 15 persons. 74SOLAS14 III/34 LSA Code 4.4.9.1 Code: 17ac
