Vessel reported a on-board fire in the engine room possibly at the hydraulic tank to SECNOLA. Vessel shall provide damage survey report and salvage, dead ship movement, and transit plan to the satisfaction of the CG prior to movements.

Piping and machinery components that exceed 220 °C (428 °F), including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Measures must be in place to prevent flammable or combustible liquid piping leaks from coming into contact with these components. Both generator prime movers were missing insulation on exhaust manifolds. Install to protect from possible Machinery space fire.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. 3 of the 4 securing pins were missing the factory designed cotter pins. Install pins as designed.

The Hailing port was not marked on the stern of the vessel. Permanently mark the hailing port at next drydock.

Information regarding chemical testing, particularly post casualty, was not found on DAPI manual onboard. Update the manual for the crew to follow for chemical post casualty testing.
