The width of corridors, doorways and stairways which form part of the evacuation paths shall be not less than 900 mm for passenger craft and 700 mm for cargo craft. This width maybe reduced to 600 mm for corridors, doorways and stairways serving spaces where persons are not normally employed. There shall be no protrusions in evacuation paths which could cause injury, ensnare clothing, damage lifejackets or restrict evacuation of disabled persons. Requirements of this paragraph do not apply to aisles (fore-aft passageways separating seating areas) or to spaces between adjacent rows of seats. However, the width of aisles and the seat pitch shall be such as to allow the craft to comply with the provisions of 4.8. Observed obstructed means of escape in MVZ4/Deck 4 Crew stairs, Perfum & Liquor Room, and Laundry Room. 74 SOLAS 2O X/3 HSC Code 4.7.13

The construction of doors and door frames in 'A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame equivalent to that of the bulkheads in which the doors are situated. Fire Doors FD04211, FD044102, FD04203, FD05303, and FD06307 did not close completely. 74 SOLAS 20 II-2/9.4.1.1.2

The emergency source of electrical power may be either a generator or an accumulator battery, which shall comply with the following: started automatically upon failure of the electrical supply from the main source of electrical power... PSCOs received report the vessels transitional UPS system had malfunctioned, resulting in the emergency generator not being able to automatically start in case of an emergency. 74SOLAS Cons. 2020 II-1/42.3.1.2

Control handles shall be provided at each side of the bulkhead at a minimum height of 1.6m above the floor and shall be so arranged as to enable persons passing through the doorway to hold both handles in the open position without being able to set the power closing mechanism in operation accidentally. The direction of movement of the handles in opening and closing the door shall be in the direction of door movement and shall be clearly indicated. PSCOs observed that after activating one of the handles for the watertight door 401 it failed to operated and close. 74 SOLAS 04 II-1/15.7.4

Every ship of 100 gross tonnage and above, and every ship which is certified to carry 15 or more persons, and fixed or floating platforms shall carry a garbage management plan which the crew shall follow. PSCO observed a garbage bin located in the theater changing area by door 05105 that was being used incorrectly by having food, plastic, metal, and paper waste products when is should only have had paper products. MARPOL 2017 Annex V/10.2

The following functional requirements shall be met escape routes shall be maintained in a safe condition, clear of obstacles. Within the medical center, PSCOs observed medical equipment incorrectly stored in the hallway leading to the exit. 74 SOLAS 20 II-2/13.1.2

The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the spaces being ventilated. The means of closing shall be easily accessible as well as prominently and permanently marked and shall indicate whether the shut-off is open or closed. Port bow thruster space ventilation fire damper does not remotely actuate. 74 SOLAS 20 II-2/5.2.1.1

PSCOs observed galley door 14613 unable to fully close. - 74 SOLAS 09 con. II-2/1.1.4.1

07199-Fire Safety- The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. Means shall be provided to limit the accumulation of flammable vapours. PSCOs observed the accumulation of diesel fuel in the engine bilge compartments of lifeboats 5, 6, 8, and 12. - 74 SOLAS 09 con. II-2/4.1.2

Prior to ship leaving port and at all times during the voyage all lifesaving appliances shall be in working order and ready for immediate use. Tender #31 STBD engine will not set to neutral.

There shall not be any doors along any designated escape route which require keys to unlock them when moving in the direction of escape. Food store deck door to aft mooring station is locked and is the second means of escape. (Deck 4 frame 4 area)

1499 - The condition of rhe ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fir to proceed to sea without danger to the ship or persons on board.
