Vessel was involved in an allision and sustained damage consisting of 2' x 12' hole IWO P/S #3 aft Wing Void. Vessel shall proceed to TT 104 Westwego, LA for tank cleaning, and onto to New Orleans Ship Yard New Orleans, LA for gas freeing of tanks and permanent repairs. Vessel owner must provide cognizant OCMI with damage survey and repair plans. Repair to the satisfaction of attending MI.

Action required: 17 - Rectify deficiencies prior to departure
Resolved 3 March 2016
Resolution: Vessel replaced port side side plate coving #4 and #3 tanks. Tested all repairs (see attached Repairs Maintenance Work List) by conducted satisfactory air test at 1.5 psi on #2, #3, and #4 wing void tanks.
