A tank vessel must have all closure mechanisms on the following openings properly closed. Multiple void hatches were missing a gasket and allowed for rain water to particially fill several voids.

All cargo tank hatches must be clearly marked in not less than three inch lettering “Danger—Keep Out”. Three cargo hatches not properly marked.

Carge hoses must be marked with the MAWP. The #2 cargo hose was not marked with the MAWP as refereneced in 33 CFR 154.500.

As per 46 CFR 111.01-1: Electric installations on vessels must ensure maintenance of system integrity through compliance with the applicable system requirements (IEEE, NEC, IEC, etc.) to which plan review has been approved. The cover for the port side electrical connection box for the liquid overfill protection device was missing exposing the electrical components to the weather; Labels (previously on the cover) were missing for: Labeled “Connector for Barge Overflow Control System” and labeled with the total inductance and capacitance of the connected switches and cabling.

As per 46 CFR 111.01-1: Electric installations on vessels must ensure maintenance of system integrity through compliance with the applicable system requirements (IEEE, NEC, IEC, etc.) to which plan review has been approved. Batteries for the liquid overfill protection device were physically damaged and no longer servicable.

Vessel fitted with overfill protection devices as described by 46 CFR 39.2003 (meter stick type) to meet 33 CFR 155.480(a). Meter sticks for devices on cargo tanks #1 & 3 were damaged (magnet detached from sticks). (705: Correct prior to loading cargo tanks.)

IAW 46 CFR 110.01-3 Electrical repairs must comply with the regulations in 46 CFR Subchapter J. Electrical junction box in starboard #1 void appeared to have been repaired using a plastic back and electrical tape.

All vessels must have draft marks plainly and legibly visible. Vessel's draft marks were not plainly visible.

Materials shall be at least equivalent to the requirements of the American Bureau of Shipping; Vessel's hatches were missing strong backs or missing a gasket.

Transfer hose required to meet 33 CFR 154.400 - marked with "Oil Service" and the Maximum Allowable Working Pressure (MAWP). 1"x50' cargo hose not marked as required.

Vessel equipped with one overfill device that is permanently installed on each cargo tank. Overfill device installed on #1 cargo tank is broken (meter stick is detached from magnet). Repair/replace level indicating meter stick and prove proper operation. Until device is operational, disable device to prevent reliance on faulty device and provide continuous visual monitoring of #1 cargo tank during loading operations.

Original COI required to be on board vessel, although not required to be "Posted". Neither the original nor a copy of the COI was on board the vessel at the time of the inspection. Copy of COI was located in an office and the original could not be produced.

Vessel deck void hatches fitted with gasket. #2 port wing void, aft hatch cover gasket missing at time of inspection. Gasket required to be replaced.

Cargo hoses required to be labeled with Maximum Allowable Working Pressure (MAWP). 1 inch cargo hose was not properly labeled with MAWP as per 33 CFR 154.500 as referenced by 33 CFR 155.800. Hose required to be properly labeled prior to using to conduct cargo transfer.

Vessel required to have oil transfer procedures (OTPs) on board. OTPs not on board or available for inspection. OTPs required to be placed on board vessel prior to next cargo transfer.

Test record for cargo (piping) relief valve and pressure gauge not available for inspection.

Vent for #3 cargo tank, fitted with a closure device, was closed rendering vent inoperable. Vent required to remain open for proper operation.

Publicly owned vessel under volunteer compliance agreement did not have records of transfer equipment tests and inspections at time of CG inspection.

Publically owned vessel's oil transfer procedures contained out of date contact information.

Barge requires a hydrostatic test of entire piping transfer system. Last Hydrostatic test conducted APR2010.

EPA Vessel General Permit missing. Vessel required to obtain and have onboard vessel.

Crop & renew Four foot by one foot fracture located in the port stern corner (box end) between the 8 and 12 foot draft mark.

No test records for cargo pressure gauge and pressure relief valve.

Two void space hatch cover gaskets severely worn.

An unathorized additional 4000 gallon slop oil tank was placed on the forward part of the barge.

Cargo piping improperly marked with test pressure and not with MAWP.

55 gallon drums improperly stored on open deck.

Vessel has 2 CO2 fire extinguishers on board, but only one is CG approved. Vessel is required by the COI to have 2 approved extinguishers on board.

Prior to loading or off loading cargo, barge must have a periodic inspection as required by 46 CFR 31.17(b). As of 20SEP2002, certificate of inspection for this barge is invalid due to the 90 day grace period for inspections expiring.

PRIOR TO TRANSFERRING CARGO, ENSURE CARGO HEADER HAS A RELIEF VALVE AND THAT IT IS PROPERLY SET.

PRIOR TO TRANSFERRING CARGO, ENSURE CARGO HEADER AND HOSES (4) ARE HYDROSTATICALLY TESTED AS PER 33 CFR 156.170.

ENSURE VESSEL HAS MATERIAL SAFETY DATA SHEETS FOR ALL CARGOES CARRIED, INCLUDING THE PRODUCT STORED IN THE 55-GALLON DECK DRUMS.
