Lifejackets must have the following attachments and fittings: (1) Each lifejacket must have a lifejacket light approved under approval series 161.012 or 161.112 securely attached to the front shoulder area of the lifejacket. (2) Each lifejacket must have a whistle firmly secured by a cord to the lifejacket. None of the lifejackets onboard had lights or whistles. 46 CFR 141.340 (g)(1) & (2)

Each exhaust pipe from an internal combustion engine which is within reach of personnel must be insulated or otherwise guarded to prevent burns. The outlet elbow coming out of the turbo chargers on both main diesel engines did not have exhaust lagging. 46 CFR 144.830

The following systems must be equipped with gauges at the machinery location: (1)Main engine lubricating oil pressure and main engine RPM; The tachometer on the STBD main diesel engine was not functioning. 46 CFR 143.230 (c)(1)

The owner, managing operator, or master of each towing vessel must maintain the required navigational-safety equipment in a fully-functioning, operational condition. The chart plotter/radar was not functioning at the beginning of the inspection. 46 CFR 140.620 (b)

Each towing vessel operating in oceans and coastwise service must have a line throwing appliance approved under approval series 160.040. The vessel only had one disposable type line throwing device.

Install guards on both main engine to prevent personal from coming into contact with the Fly wheel and the belt.

Replace non operational rpm gauge on starboard main engine.

Prove proper operation of the following alarms. (1) Main engine low lubricating oil pressure; (2) Main engine high cooling water temperature; (3) Auxiliary generator engine low lubricating oil pressure; (4) Auxiliary generator engine high cooling water temperature; (5) Low hydraulic steering fluid levels, if applicable; and (6) Low fuel level, if fitted with a day tank.

Any portable pump must have sufficient hose length to reach all space on the vessel to be dewatered. Vessel current portable pump is bolted down and doesn’t have a long enough hose.

All fire-detection systems must be inspected or tested at least once every 12 months, as prescribed in paragraphs (a)(1) through (8) of this section. The fire detection system was not inspected within the past 12 months and was displaying “system trouble alarm”, have fire detection system inspected. 46 CFR 142.240(a)

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. The life raft hydrostatic release unit was found broken and installed incorrectly to the painter line and securing strap, replace life raft hydrostatic release. 46 CFR 141.230

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. An unapproved welded repair was conducted to the pressure vessel, remove pressure vessel from service and provide repair plan for replacement pressure vessel to satisfaction of Marine Inspector. 46 CFR 143.205(a)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing emergency conditions. A generator was installed on deck to supply power to the vessel’s switchboard via the shore-tie connection plug. The switch board installed onboard is only setup with a 50 amp breaker for the machinery space generator and 50 amp breaker for shore tie connection. Provide documentation on when the installation of the generator was installed outside the machinery space and documentation on the generator rating. 46 CFR 143.205(a) 21AUG2024:Electrical drawings from inital construction were provide which indicate the generator outside was not installed at new construction but was completed prior to Subchapter "M" impltementaion. The generator will be considered as exsisting equipment. The operator has contracted an engineering firm to provide plans to permenatly wire the generator to the switch board in lieu of the the shore tie connection from the generator. Deficeincy has been extended to next scheduled dry dock period, 30JUN2025. Operator to provide P.E. approved electrical drawings for verification during dry dock period.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The bilge/fire pump is connected by a 50 amp breaker and the machinery space generator is protected by a 50 amp breaker. Provide documentation on the bilge/fire pump motor rating and the machinery space generator rating. 46 CFR 143.2205(a) 12JUL2024: The bilge/fire pump is rated at 18 AMPs, replace 50 AMP breaker at switchboard with appropriately rated breaker.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided: (6) Low hydraulic steering fluid levels. The hydraulic steering gear system does not have an alarm at operating Station. 46 CFR 143.230(a)(6)

The following systems must be equipped with gauges at the machinery location: (1) Main engine RPM; (3) Auxiliary generator engine lubricating oil pressure and auxiliary generator engine RPM; (4) Auxiliary generator engine cooling water temperature. Main engine and auxiliary generator engines did not have gauges locally, install gauges locally. 46 CFR 143.230(c)

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light approved under approval series 161.010 or 161.110. The floating electric water light had the incorrect battery installed per manufactures instructions, replace battery with manufacture required battery. 46 CFR 141.360(c)(2)

Nonmetallic flexible fuel hose is acceptable if it: (i) Is used in lengths of not more than 0.76 meters (30 inches); (ii) Is visible and easily accessible. The generator installed outside of the machinery space is being supplied fuel from nonmetallic flexible fuel hose approximately 20’ in length and located behind shelving. Provide repair proposal for permanent installation of fuel piping for the generator. 46 CFR 143.265(d) 30JUL2024: Received repair plan to replace exisiting non-metalic flexible fuel hose with seamless steel tubeing and synthetic rubber with an inner tube or reinforced with wire braid hoses. Extended deficiency due date unitl 30JUN2025 during upcoming scheduled dry dock period.

Each towing vessel must be fitted with a general alarm. There is no general alarm on board to notify crew in event of an emergency, provide repair plan to install general alarm as per 46 CFR 143.235 to satisfaction of Marine Inspector. 46 CFR 143.235

Controls may not be located in any space that could be cut off from the operator in the event of fire in the protected space. A Sea-Fire Model H240 clean agent system was installed on board with the releasing station installed in the protected space. Provide a repair plan to relocate the release station outside the space the system protects to satisfaction of marine inspector. 46 CFR 136.110 46 CFR 95.16-5 26JUL2024: Received repair plan to relocate remote release control outside machinery space. Extended deficeincy due date to 30JUN2025.

Vessel does not have a line throwing appliance approved under 160.040 or auxiliary line.

Installed bilge pump for emergency dewatering is unable to take suction.
