Hydraulic power-operated steering gear shall be provided with the following: a fixed storage tank having sufficient capacity to recharge at least one power actuating system including the reservoir, where the main steering gear is required to be power-operated. The storage tank shall be permanently connected by piping in such a manner that the hydraulic systems can be readily recharged from a position within the steering gear compartment and shall be provided with a contents gauge. Electrical equipment and wiring shall not be fitted in enclosed cargo spaces or vehicle spaces unless it is essential for operational purposes in the opinion of the Administration. However, if electrical equipment is fitted in such spaces, it shall be of a certified safe type***** for use in the dangerous environments to which it may be exposed. The electrical wiring, junction boxes, and explosion-proof lighting fixtures located throughout the port and starboard cargo discharge tunnels are severely degraded, broken, or missing, compromising the explosion-proof rating of these spaces and creating an active ignition source in a hazardous atmosphere. 74 SOLAS (2020) II-2 / 19.3.2

Pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the spaces being ventilated. The means of closing shall be easily accessible as well as prominently and permanently marked and shall indicate whether the shut-off is open or closed. The permanent, approved weathertight and fire-closing cover for the portside aft superstructure HVAC supply intake has been removed to facilitate the routing of temporary flexible ventilation ducting. The temporary closure installed around the ducting (consisting of thin sheetmetal) does not meet the structural strength or fire-rating requirements of closure. 74 SOLAS (2020) II-2 / 13.1.2

For all ships the means of embarkation and disembarkation shall be inspected and maintained** in suitable condition for their intended purpose, taking into account any restrictions related to safe loading. The steel deck-mounting structures and securing brackets for the port and starboard embarkation ladders show signs of severe corrosion. 74 SOLAS (2020) II-1 / 3-9.3

Manholes and flush scuttles in position 1 or 2 or within superstructures other than enclosed superstructures shall be closed by substantial covers capable of being made watertight. Unless secured by closely spaced bolts, the covers shall be permanently attached. The main deck watertight manhole covers for BWT's were observed to be missing multiple securing studs and bolts. BWT sounding tubes could no longer be secured. ICLL 1966 Annex I / II.18.1

Efficient guard rails or bulwarks shall be fitted on all exposed parts of the freeboard and superstructure decks. The height of the bulwarks or guard rails shall be at least 1 metre (39½ inches) from the deck, provided that where this height would interfere with the normal operation of the ship, a lesser height may be approved if the Administration is satisfied that adequate protection is provided. The portside weather deck guard railing is missing a section. ICLL 1966 Annex I / II.25.2

The purpose of this regulation is to provide means of escape so that persons on board can safely and swiftly escape to the lifeboat and liferaft embarkation deck. For this purpose, the following functional requirements shall be met:escape routes shall be maintained in a safe condition, clear of obstacles. Temporary flexible HVAC ventilation ducting was observed running through the aft exterior ladder well. This obstruction prevents rapid evacuation during an emergency. 74 SOLAS (2020) II-2 / 13.1.2

Electrical installations shall be such that the safety of passengers, crew, and ship from electrical hazards will be ensured. The electrical installations on the main weather deck are degraded and present active shock and fire hazards to the crew. The cable sheathing in numerous areas is frayed and cracked, exposing the inner insulated conductors. 74 SOLAS (2020) II-1/40.1.2

The company should ensure that inspections are held at appropriate intervals (10.2.1) and records of these activites are maintained (10.2.4). As noted on deficiencies numbered 2, 3, and 11, the vessel shows evidence of a systematic failure of their Safety Management System (SMS), specifically regarding maintenance and operational procedures. The following functional requirements shall be met: Fire-Extinguishing appliances shall be readily available. Fir detection systems are serviced annually, but the crew does not have means to conduct functional tests onboard between services. A record shall be kept to the satisfaction of the Administration and as required by the Radio Regulations of all incidents connected with radiocommunication service, which appear to be of importance to Safety to life at sea. Cables shall be installed and supported in such a manner as to avoid chafing or other damage. The electrical cables for sensing devices in the fuel oil purifier room are melting due to contact with hot surfaces. Cite: SOLAS74(04) II-1/5.5 Action Taken: 17ac

The condiction of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. The machinery and associated piping systems and fittings shall be of a design and construction adqeuate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons onboard, due regard being paid to moving parts, hot surfaces and other hazards. The following functional requirments shall be met: the ignitablity of combustible materials shall be restricted. The port side engine room tank top deck overhead pipe has a drip of bunkering fuel, which is imprequating its insulation. Cite: SOLAS74(04) II-2/4.1.3 Action taken: 16ac

The Administration shall take approriate steps to ensure uniformity in the implementation and application of the provisions of this part in respect of electrical installations. Shipboard incineration of polyvinyl chlorides (PVCs) shall be prohibited, expect in shipboard incinerators for which IMO Type approved certifications have been issued. The current incierator was built in 2005. However, the vessel's SMS section 08.01.08 "Garbage Incineration" states the following items are prohibited at anytime: polyvinyl chloride plastics (PVCs) and is only allowed in new incinerators that meet the requirments of regulation 16.6.1 of MARPOL Annex VI. Fire-Fighting systems and appliances shall be kept in good working order and readily available for immediate use. Emergency fire pump was not immediately available for use and took over 45 minutes to start the emergency fire pump. Cite: SOLAS74(04) II-2/14.2.1.2 Action Taken: 60ac/10c

01-11199-Life saving appliances shall be stowed ready for immediate use. The port quarter life ring was blocked by on deck consumables for ballast system. SOLAS 74/14 CON Chp. III Reg. 20.2 - 10C

02=08199-An audible and visual alarm for high water must be fitted in the FPT of a bulk carrier. The FPT alarm is not operational. SOLAS 74/14 CON Chp. XII Reg. 12.1.3 - 40AC

04-07106-Fire detection shall be provided suitable for the nature of the space. Fire detection in the purifier room has exposed wires. SOLAS 74/14 CON Chp. II-2 Reg 7.1.1

05-08199-Bulk carriers shall be fitted with audio and visual alarms in each cargo hold for water ingress. Cargo hold #1,2,5,and 6 alarms are not operational. SOLAS 74/14 CON Chp. XII Reg. 12.1.1

The equipment shall be maintained to conform with the provisions of this annex and no changes shall be made in the equipment, systems, fittings, arrangements or material covered by the survey, without the express approval of the Administration. The direct replacement of such equipment and fittings that conform with provisionsof this annex is permitted, PSCO noted exhause scrubber and new fuel on board is not compliant with IAPP supplement.

The sulfur content of any fuel oil used on board ships shall not exceed the following limits: 0.50% m/m on and after January 1, 2020. Vessel reported that their scrubber system for their exhaust malfunctioned while in transit to Thunder Bay. They had to switch over to non-compliant IFO380 fuel for the remainder of their transit.
