THE CONDITION OF THE SHIP AND ITS EQUIPMENT SHALL BE MAINTAINED TO ENSURE THE SHIP IS FIT TO PROCEED TO SEA WITHOUT DANGER TO PERSONS ON BOARD. WHILE AT THE PORT FACILITY, PSCO OBSERVED TWO EXPLOSIONS AND HEAVY SMOKING FROM E/R EXHAUST STACK. AFTER INTERVIEWING CREW, IT WAS DETERMINED THE EXPLOSIONS OCCURRED IN THE SHIPS BOILER SHORTLY AFTER MAINTENANCE WAS PERFORMED BY THE CREW. THE INCIDENT RESULTED IN INJURIES TO SECOND ENGINEER, WHO WAS REMOVED FROM THE SHIP FOR MEDICAL TREATMENT. 17(a)(c).
