Administrations shall require that records of daily hours of rest be maintained, to allow monitoring and verification of compliance. PSCO obtained objective evidence, in the form of written statements, that crew work/rest records were being falsified to hide violations of STCW standards by direction of the vessel's master which resulted in an individual assigned to a navigational watch not being given proper rest and subsequently falling asleep on watch. The company and the ship shall comply with the requirements of the International Safety Management Code. Objective evidence discovered during an expanded PSC Exam revealed the following non-conformities: The vessel failed to fully implement the requirements of the ISM Code through their safety management system procedures as evident by the following deficiencies indicating that the ship and or company are not meeting the SMS requirements. An SMS audit is required prior to release from detention. 74SOLAS (20) IX/3.1 ; ISM Code Part A #1: Objective evidence (cited on Form B items 2-15) exists that the vessel is not implementing the Safety Management system (SMS). The vessel has demonstrated a systematic lack of maintenance of critical equipment. Vessel's lack of SMS documentation and long-standing deficiencies identified throughout the course of the Port State Control Examination were not documented for any nonconformity, near miss or hazardous occurrence according to the vessel's Safety and Quality Management System. 74 SOLAS (20) IX/3.1; ISM Code Part A/10

All access openings in bulkheads at the ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the un-pierced bulkhead and weathertight when closed. PSCO observed severe corrosion on the upper edge of entry door and seal to carbon dioxide room, jeopardizing the weather tight integrity. ILLC Reg 12(1)

Resolution: The door and seating area were mechanically cleaned. Door was removed and three areas, approximately 30 cm each, of knife-edged outer ring was cropped and renewed. The door was reinstalled with a new gasket and tested to the surveyor's satisfaction.

Resolution: The missing light was found in the main deck storeroom with the light housing separated from the mounting bracket. Mounting bracket was re-secured to the light housing, re-mounted to the starboard side pilot embarkation davit, and operated all sat.
