The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. PSCO determind that while getting underway the engineer on watch had No. 1 Main Start air isolated and when low pressure alarm for No. 2 Main Start Air went off, the engineer opened the cross valve which subsequently emptied No. 1 causing another low pressure alarm. It was subsequently identified that only air compressor No. 2 was in standby. 74 SOLAS 20 II-1/26.1

The Company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the Company. The operational nature of the deficiency noted above provides objective evidence of a serious failure of the implementation of the ISM Code. While questioning the Engineering department related to deficiency No. 1, PSCO found the "presailing check list" was not completed prior to getting underway from Belle Chasse Anchorage. When C/E was asked what the "plant operating instructions" on action 8 of the presailing check list was, C/E could not provide and was unfamiliar. Request external audit is conducted within 90 days. ISM Code 10.1

PSCO observed a fuel oil leak on the number 5 cylinder of the Main Diesel Engine, 02 buckets placed under the fuel oil transfer pump to catch fuel, and an accumulation of oil sludge in the bilge 74 SOLAS 20 II-2/4.1.1 - 17ac
