An Immersion suit of appropriate size complying with the requirements of section 2.3 of the code shall be provided for every person on board the ship. PSCO observed that the immersion suits on board were severely deficient in their material condition. The suits' seams between the front zipper panel and inner layer of thermal protective insulation were delaminated due to a failure in the adhesive. The adhesive was also beginning to fail in the seams between the front zipper panel and the outer shell. A total of 33 out of the 43 immersion suits were found in this condition.

The company should establish procedures, plans, and instructions, including checklist as appropriate, for key shipboard operations concerning the safety of the personnel, ship, and protection of the environment. The various tasks should be defined and assigned to qualified personnel. Immersion suit monthly inspection checklist requires internal and external inspections to evaluate the suits for damage, rips, tears, or punctures. PSCO observed at least 20 suits that were obviously deteriorated. Vessel's SMS requires that monthly inspections of lifesaving equipment be completed using the checklist incorporated in the SMS. The monthly checklist was not used during a monthly inspection for NOV 2018. Vessel's SMS requires that monthly inspections of lifesaving equipment be logged in the vessel's deck log. The inspection of the vessel's immersion suits, completed on Nov 18, 2018, was not logged in the vessel's deck log book. Due to the PSCO's observations of these con-conformities, an audit of the vessel's SMS by the certificate issuing authority is recommended to be completed within 30 days.
