Resolution: PSCO observed new wire installed to emergency diesel generator fuel shut off valve. Upon satisfactory observation of operational test of fuel shut off valve, deficiency cleared.

The company and the ship shall comply with the requirements of the ISM Code. The company should ensure that each ship is manned with qualified, certificated and medically-fit seafarers in accordance with national and international requirements; and appropriately manned in order to encompass all aspects of maintaining safe operations on board. Vessel Master who reported 04DEC2019 was relieved of duties after being found intoxicated while getting vessel underway. Vessel currently does not have master onboard to meet safe manning requirements. Cite: 74SOLAS(14) IX/3.1, ISM CODE 6.2. 74SOLAS(14)V/14.2. Code 60b.
