A 12 inch crack on the side plating by the #1 starboard wing void. Vessel allowed to offload cargo. After applying temporary repairs and #1 starboard wing void is dewatered, vessel is allowed one time transit to Channel View, TX to make permanent repairs.

Resolution: Witnessed satisfactory testing on (08) shell inserts as described in activity. Air/Soap test at 1 PSI all sat.

Conducted inspection of vessel, during the document review found the vessel didn't have a valid Certificate of Finanacial Responsibility. Several Declaration of Inspections revealed vessel conducted cargo operations without valid Certificate of Financial Responsibility.

Vessel ran aground near ICW MM 56, Bay St. Louis, MS. Vessel was allowed to continue her transit to Nustar in St. James, LA; however, the vessel must be inspected by local OCMI prior to departure from that terminal.

Resolution: Marine Inspectors attended vessel found minimal damage in the bow rake foreword of the collision bulkhead, no broken welds. One tripping bracket bent. Vessel rep will make permanent repairs prior to receiving periodic inspection endorsement in Feb 2015.
