For tankers of 20,000 tonnes deadweight and upwards, a fixed deck foam fire-extinguishing system shall be provided complying with the provisions of the Fire Safety Systems Code.
The distance from the monitor to the farthest extremity of the protected area forward of the monitor shall not be more than 75% of the monitor throw in still air conditions.
Vessel was unable to produce adequate throw due to crew proficiency.

Vessel's #3 fuel pump on main diesel engine was inoperable. This caused a loss of propulsion.

PSCO observed improperly installed electrical wiring in the lighting in the vessel's pump room posing and electrical hazard.

The company should ensure that the policy is implemented and maintained at all levels of the organization, both ship-based and shore-based. The vessel's SMS 1.2.13 "Electrical Maintainence" states electrical equipment must be properly maintained and kept arc free. Based on the numeroius electrical issues found on the vessel, reccomend an audit of the vessel's SMS in regards to electrical maintenance.
