In a ship in which oil is used, the arrangments for the storage, distribution, and utilization ofthe oil fuel shall be such as to ensure the safety of the ship and persons on board. Fuel oil was leaking from the #1 and #2 fuel supply pumps, creating a fire hazard.

Where necessary for the normal operation of the ship chains fitted between two stantions and/or balwarks are acceptsble in lew of guard rails. PSCO observed safety chains at the port side embarkation point near cargo hold #4 to e affixed by rope, creating a hazard for personnel.

Courses shall be not more than 380mm apart. PSCO observed a railing next to the gangway on the starboard side of the vessel where the lowest course had been bent downwards, causing the gap between the lowest and middle course to be greater than 380mm.

The height of the balwarks or guard rails shall be at least 1M from the deck. PSCO observed a railing between the #1 and #2 cargo holds on the starboard side of the vessel had been severly dented, making it below the required height.

The function of fixed fire and fire alarm systems shall be periodically tested to the satisfaction of the administration. PSCO observed crewmember using an open flame to test a flame detector. Approved means of testing flame detectors was broken. Ship has requested a new heat lamp from company in September of 2021, but has yet to receive it.

0695 - Operations/ Management - All engines in the lifeboat and rescue boats shall be ran for a total of not less than 3 minutes provided the ambient temperature is above the minimum temperature required for starting the engine. Crew states that during weekly inspections of the rescue boat the engine is only ran for 30 seconds without water until it shuts off. 74SOLAS(14) III/20.6.2

Life saving appliance maintenance manual states that the rescure boat shall always be started with a cooling water source. Crew states that during weekly inspections water is not used while running the engine
