Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines at the tank connection. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The vessel's port fuel shutoff valve was not operable. Service and prove proper operation of the port fuel shutoff valve.

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Remote fuel shutoff direction of operation missing.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked "First-Aid Kit". A first-aid kit must be easily visible and readily available to the crew. First aid kit is incomplete. Provide USCG-approved kit or a kit that complies with the list in 46 CFR 160.041.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training and general description of the drill scenario and training topics. Drill log provided does not provide a description of the drills.

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Wing nut are being used to connect to battery terminals.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date. U.S. Coast Pilot or similar publication. Missing U.S. Coast Pilot.

Code 07199: Shut-off valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. Both port and starboard fuel shut-off valves were found no closing as designed, and handle could be turned 360 degrees to both sides. 46 CFR 182.455 (4), Code 17.

Code 08199: On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in a space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. Port and starboard bilge alarm float switch was found not to be secured to any internal structure the vessel. 46 CFR 182.530 (a) (3), Code 17C.

Code 18299: No marine employer shall engage or employ any individual to serve as a crew member unless the individual passes a chemical test for dangerous drugs for that employer. Vessel was found to be in operation with crew members without evidence of being tested prior starting to work, and no proof of pre-employment testing was provided. 46 CFR 16.210(a). Code 17C.

Code 07110: Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Certification or licensing by a state or local jurisdiction as a fire extinguisher servicing agency will be accepted by the Coast Guard as meeting the personnel certification requirements of NFPA 10 for annual maintenance and recharging of extinguishers. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility must perform the required inspections, maintenance procedures, and hydrostatic pressure tests. A tag issued by a qualified servicing organization, and attached to each extinguisher, may be accepted as evidence that the necessary maintenance procedures have been conducted. One of three portable fire extingushers did not had evidence of any testing, maintenance or inspection from a reputable fire technician. Provide servicing report. 46 CFR 176.810, code 16C.

Code 11118: Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Provide a lifejacket instruction donning placard for the type and carried on board. 46 CFR 185.516, Code 16C.

Code 18299: The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in a Serious Marine Casualty. No alcohol testing device was available. 46 CFR 4.06-15 Coe 16C.

Code 03105: At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Port and starboard fuel tank covers were found with a broken hinges and without securing devices to prevent them from opening during use. 46CFR176.830 Code 16C.

Code 10116: A vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. Vessel was missing updated required publications. 46 CFR 184.420, Code 16C.

Code 08199: On a vessel of at least 26 feet in length or more, a visual and audible alarm must be provided at the operating station to indicate a high water level. The high bilge water visual light alarm was found inoperable. 46 CFR 182.530 Code 16C.

Code 02108: At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Electrical wiring was found hanging and partially submerged in the bilge water in both port and starboard aft compartments. 46 CFR 176.830 Code 16C.

Code 02108: Electrical equipment on a vessel must be installed and maintained to Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Multiple dead-end wires were found underneath the main operating center console. 46CFR183.200, Code 16C.

Code 05109: A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel VHF/GPS Integrated Radio was found not displaying coordinates associated with the DSC; and the amp meter batter was found discharged. 46 CFR 184.502 & 47 CFR Part 80 FCC. Code 16C.

Code 10109: All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. The all around light located on top of the canopy was found inoperative. 46 CFR 183.420 Code 16C.

Code 99101: Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. One out of two portable light was found inoperative. 46CFR184.430

Code 03105: A hatch exposed to the weather must be watertight, except that the following hatches may be weathertight: on a vessel that operates only on protected waters. Water ingress from rain, and wash-down during clean-up was found to enter through all inspection port hatches. This deficiency shall be corrected during next dry dock to be scheduled not later than August 2024. 46CFR179.360, Code 705: for next dry dock.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Vessel's life ring buoy grab line deteriorated and in need of replacement. Replace. 46 CFR 176.808(b)

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Stbd aft bilge alarm light is not operational. Repair. 46 CFR 182.530(a)

The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The vessel's stbd fuel shutoff valve extends past the closed position when pulled. Provide a means to ensure the valve remains in the closed position when pulled. 46 CFR 182.455(b)(4)

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. An examination of the vessel's documentation revealed that an individual serving as a crew member is not under the owner's employment and not found on the vessel's maritime consortium list of crew. This particular individual served as a crew member on 18 occasions as evidenced in the vessels' Waiver & Release form.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. The starboard high water bilge indicator sounded an alarm without displaying a visual alarm at the operating station, and the port side high water bilge indicator failed to display both a visual and audible signals at the operating station.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests; portable fire extinguishers must be inspected and maintained in accordance with NFPA 10; certification or licensing by a state or local jurisdiction as a fire extinguisher servicing agency will be accepted by the Coast Guard as meeting the personnel certification requirements of NFPA 10 for annual maintenance and recharging of extinguishers. Portable fire extinguisher maintenance records or certification was not found on board the vessel.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. The first-aid kit onboard was missing several items required by 46 CFR Table 160.041-4(b) and was missing the instructions for use.

Each life jacket must be marked in clearly legible block capital letters with the vessel's name. Found 18 of the required 44 life jackets missing the vessel’s name. Mark appropriately.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Each ring life buoy on a vessel must be approved in accordance with § 160.050 in subchapter Q of this chapter. The grab line shall be 3/8-inch diameter polyethylene, polypropylene, or other suitable buoyant type synthetic material having a minimum breaking strength of 1,350 pounds. The current grab line was found to be degraded due to UV sun damage.

A vessel not required to have a power driven fire pump, must have at least three 9.5 liter (2 1/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. One of the provided fire buckets did not have a lanyard attached due to a broken handle.

The official number of the vessel, preceded by the abbreviation “NO.” must be marked in block-type Arabic numerals not less than three inches in height on some clearly visible interior structural part of the hull. The number must be permanently affixed to the vessel so that alteration, removal, or replacement would be obvious. The vessels’ official number is displayed at the top of the operating station with the numbers in white vinyl tape over black vinyl tape.

Each portable fire extinguisher must be of an approved type and the minimum number required by table 181.500b. Per this table the vessel shall be required to carry 03 fire extinguishers. Vessel had 02 fire extinguishers on board at time of inspection. 46CFR181.500(a)

Portable extinguishers must be inspected and maintained IAW NFPA 10 for annual maintenance and recharging of extinguishers. The 02 fire extinguishers on board were both found past due for their annual servicing. 46CFR176.810(b)(1)

Man overboard drills and training shall be logged for review by the coast guard. Vessel logs were not available for inspection on board vessel. 46CFR185.520(f)

Repair/replace 01 adult life jacket. Confirm retro tape is present.

Provide placard for life jacket model carried on board vessel.

Stencil in black lettering the words "FIRE BUCKET" onto 03 fire buckets.
