No vessel may be navigated unless it has its service and on board the crew complement as required by the COI. During ACP COI oversight found the vessel did not have required Able Body Seaman on board. Vessel is required to submit a request for sailing short or attest to meeting minimum manning on valid COI prior to departing the Port of San Diego. Cite: 46 CFR 15.515

Ships shall be designed, constructed and maintained in compliance with the structural, mechanical and electrical requirements of a classification society which is recognized by the administration. Ship reported that there is a hydraulic leak from the upper steering gear to rudder stock seal. Repair the steering gear's upper seal in accordance with OEM specifications. Repairs shall be to the satisfaction of attending marine inspector or the vessels RO surveyor.

The Machinery shall be of design and construction adequate for the service for which they are intended. Vessel suffered a reduction in propulsion from a failed Start Air Valve for the #2 Cylinder. Vessel to prove proper operation of the propulsion engine.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Vessels emergency fire pump was able to maintain adequate pressure, however was leaking significantly from pump seal. 74 SOLAS (09) II-2/14.2.1.2

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power required by regulation II-1/43. The vessel's forward port side emergency liferaft embarkation station is not illuminated. 74 SOLAS (14) III/11.4

Not less than two lifebuoys shall be provided with lifebuoy self-activating smoke signals complying with the requirements of paragraph 2.1.3 of the Life Saving Appliances Code and be capable of quick release from the navigation bridge. Approved lifesaving appliance means carrying an approval granted by the Commandant under subchapter Q (46 CFR 159-165) of this chapter. 74 SOLAS (14) III/7.1.3 U.S. Supplement to DNV GL Rules for Classification of Ships and International Conventions 3.6.2.1

An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. In accordance with U.S. Coast Guard Marine Safety Information Bulletin 09-20 "Vessel Inspections, Exams, and Documentation", the vessel was issued a temporary COI based upon acceptance of objective evidence from the vessel's owner and/or operator attesting to the vessel being fit for its intended route and service. The vessel's owner or operator shall ensure that the vessel completes a USCG Marine Inspector attended COI Renewal inspection within 90 days of issuance of this CG-835V requirement. Failure to complete a USCG Marine Inspector attended COI Renewal inspection may result in the vessel being restricted from carrying cargo. 46 CFR 176.404

The condition of the ship and its equipment shall be maintained to conform with the provisions ofthe present regulations to ensure the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. The crew reported that the pump bearing for the s/w cooling pump outfitted with the engine room emergency bilge suction caught on fire rendering the pump non-operational.

Oil fuel pipes, which, if damaged would allow oil to escape from a storage, settling or daily service tank having a capacity of 500 L and above situated above the double bottom, shall be fitted with a cock or valce directly on the tank capable of being closed from a safe position outside the space. The fuel quick closing valve assembly in the 5 deck fire control station is leaking air past the systems relief valve. 74SOLAS(14)II-2/4.2.2.3.4

An alarm system shall be provided indicating any fault requiring attention. Main Engine Cylinder Piston Cooling Flow Sensor is not operating as designed. 74SOLAS(09)II-1/51.1

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: Structural fire protection, including fire-resisting divisions, and protection of openings in these divisions. Opening in A-class fire division of fire door BL6 found not in good working order, with the latching mechanism preventing full closure of the door. 74SOLAS(09)II-2/ 14.2.1.1.1

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures shall satisfy the following requirements: (.1) the doors shall be self-closing. (.4) hold-back hooks not subject to central control station release are prohibited. Galley A-60 door found with hold-back and unable to self-close. 74SOLAS(09)II-2/9.4.1.1.4

Your vessel is prohibited from transiting in any port within the Sector Honolulu COTP Zone without the assistance from towing vessels of adequate horsepower and maneuverability. Such assistance from towing vessels must be under the direction and control of an individual holding a Merchant Marine Credential (MMC) authorizing them to engage in assistance towing under the provisions of 46 CFR 11.482. 74SOLAS(14) II-1/29.1

[name withheld] reported a main engine slow down activation due to a deviation alarm for exhaust gas #3 cylinder while underway 400 nm from Honolulu, HI. Crew members removed the exhaust vale and identified a fractured fuel injector. [name withheld] reported that the exhaust valve and forward/aft fuel injectors were replaced in kind. Operator shall provide an ACS/RO report to OCMI attesting to repair of main engine. Master shall ensure adequate size and hp tugs are alongside when transitting in Honolulu Harbor prior to receiving ACS/RO report attesting to repair.

The company and the ship shall comply with the requirements of the International Safety Management Code. (ISM Code 6.3) The company should establish procedures to ensure that new personnel and personnel transferred to new assignments related to safety and protection of the environment are given proper familiarization with their duties. Instructions which are essential to be provided prior to sailing should be identified, documented and given. (ISM Code 7) The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. Vessel has no procedures, checklists, or familiarization requirements to operate the Oily Water Separator and no personnel on board were able to operate the OWS. Familiarization training is also required as per MEPC.107(49) Paragraph 1.1.5. Objective evidence indicates a serious failure of the implementation of the SMS, an SMC external audit must be carried out by the RO prior to this ship being released from detention. 74 SOLAS (2014 Consl.) IX / 3 ISM Code (2014) 6.3 and 7

After any survey of the ship under regulations 7, 8, 9 or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the Administration. For this purpose, the following functional requirements shall be met: Means shall be provided to control leaks of flammable liquids. MIs identified fuel leaks on #3, #4, and #5 cylinders on main diesel engine. 74 SOLAS (2014) II-2/4.1.1

Except as provided in paragraphs 3.4.4 and 3.4.5, each lifeboat shall be launched, and maneuvered in the water by its assigned operating crew, at least once every three months during an abandon ship drill. MIs unable to witness proper operation of the lifeboats during the ACP Annual Inspection. Crew shall conduct operational tests of port and starboard lifeboats. 74 SOLAS (2014 Consl.) III/19.3.4.3

Maintenance, testing, and inspections shall be carried out based on the guidelines developed by the Organization and in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. Blow dry compressed air or nitrogen through the discharge piping or otherwise confirm the pipe work and nozzles are clear of any obstructions. This may require the removal of nozzles, if applicable. MSC.1/Circ.1432 8.1.2 74 SOLAS (2014 Consl.) II-2/14.2.2.1

The purpose of this regulation is to contain a fire in the space of origin. For this purpose, the following functional requirements shall be met: the fire integrity of the divisions shall be maintained at openings and penetrations. MIs identified multi-cable transits (MCTs) which were not correctly installed or tightened on the bulkhead of the engine control room and entering the paint locker. 74 SOLAS (2014 Consl.) II-2/9.1.3

Without closing the CO2 system manually, CO2 would continue to enter the space activated for and all CO2 would be exhausted from low pressure reservoir. 74 SOLAS (2014) II-2/9.1.3

Instructions for on-board maintenance of life-saving appliances complying with regulation 36 shall be provided and maintenance shall be carried out accordingly. MI's identified a significant gap between the wedge and new wire rope terminal end and wire rope was replaced not by a manufacturer's representative. 74 SOLAS (2014 Consl.) Oil filtering equipment referred to in paragraph 2 of this regulation shall comply with paragraph 6 of this regulation. Means of control shall be provided for opening and closure of skylights, closure of openings in funnels which normally exhaust ventilation and closure of ventilator dampers. MI witnessed ventilation dampers not fully closing in the following locations: Aft outboard engine room, all starboard side shell cargo decks natural ventilation, and 05-03 on main deck forward ventilation for cargo hold. 74 SOLAS (2014) II-2/5.2.2.1

Flammable liquid lockers shall be protected by an appropriate fire-extinguishing arrangement approved by the administration. In all spaces protected by a carbon dioxide system, except cargo spaces, stopping of the ventilating fans is to be automatically actuated upon operation of the carbon dioxide system. Ventilation shutdown for paint locker did not automatically shut down upon actuating the pressure switch. 74 SOLAS (2014) II-2/6.3.2, US Supplement to DNV Rules (Rev 11)

The main engine overspeed trip circuitry is improperly activating causing the main engine to shut down before max parameters are met. USCG is requiring Lyngso Marine Tech Rep to attend and truoubleshoot/repair prior to departure from next port in LA.

Provide an approved PSTP to conduct the main engine overspeed trip automation testing iaw the Manufactures testing procedure. Current procedure does not allow testing to be done unless vessel is underway.

Starboard side deck mark shall be painted in a contrasting color to the hull.

Found stuffing tube missing on junction box for starboard lifeboat limit switch. Electronics in the junction box were found corroded. Clean electrical connections and replace stuffing tube material.

General alarm speaker located in the vicinity of the forward mooring deck is inoperable. Make repairs to general alarm speaker and complete satisfactory operational test of equipment.

Found cracked welds on support bracket for low pressure CO2 distribution piping located under the stairwell in the low pressure CO2 space. Submit weld repair proposal to OCMI for review and repair piping support brackets once weld procedures are approved by OCMI.

Verify approximate distance for measurement location to determine limit switch cut off for lifeboat davits. Prove proper operation of limit switch with minimum distance from stowed position.

Submit PSTP for steering gear loss of phase alarm that complies with 46 CFR 61.40.

Make permanent repairs to hydraulic leaks on the steering hydraulic system piping located between the filter and DCV.

Conduct a dynamic winch brake test on the starboard lifeboat and submit proof of test to OCMI.
