A cargo tank of the tank vessel connected to a vapor collection system must be equipped with a permanent or portable cargo gauging device that is a closed type as defined in 46CFR151.15.10 that does not require opening the tank to the atmosphere during cargo transfer 2) has an indicator for the level of liquid in the tank that is located where cargo trasnfer is controlled. PSCO observed an inoperable radar sensor on the port side cargo tank (number 6) in the cargo control room ullage and temperature sensor, both, gave an incorrect reading.

Each liferaft or group of liferafts shall be stowed with a float-free arrangement complying with the requirements of paragraph 4.1.6 of the Code, so that each floats free, if inflatable, inflates automatically when the ship sinks. PSCO observed paint over the holding straps for the liferaft. Configuration would have hindered the release for the hydrostatic unit.

Resolution: C/O removed built up paint from the holding straps and demonstrated that the hydrostatic release would be capable of releasing the liferafts.

Oil fuel pipes which if damaged would allow oil to escape from a storage, settling, or daily service tank having a capacity of 500L and above situated above the double bottom shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated.
Air supply line to pneumatically actuated quick closing valves (qty 02) located on diesel oil service tank found disconnected rendering valves inoperable. PSCO observed air supply line reconnected to quick closing valves but unable to verify operation of quick closing valves due to ship operating on diesel oil within the ECA.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the Company.
Minerva Marine Inc. Placard SQ/57 states "Warning. Do not obstruct, block, or tie-up open the quick closing valves and their remote controls." This placard was found posted near every oil quick closing valve in the machinery space.
Deficiency #1 provides objective evidence that the requirements of the relevant rules (to have quick closing valves capable of being operated from outside the space) and the company (to not obstruct quick closing valves and their remote controls) are not met.
