Objective evidence discovered during an expanded ISM exam revealed the following nonconformities: The vessel failed to fully implement the requirements of the ISM Code through their SMS procedures as evident by the following deficiencies indicating that the ship and/or company are not meeting the SMS requirements. Recommend an external audit. 1) The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. 2) In meeting these requirements, the company should ensure that: inspections are held at appropriate intervals. 3) [Extinguishers] shall be recharged or renewed if the loss of gas by weight exceeds 10% of the original charge… 4) The following guidelines… provides guidelines for conducting maintenance for the checklists for- Weekly, Monthly, Quarterly, Semiannual, and annually board. The Vessel’s monthly test and inspection routines checklists states ‘fire extinguishers check date of service and gauge reading to ensure they are full’. All three of the fire extinguishers in the engine room were insufficiently charged and the next closest fire extinguisher, located in the adjacent workshop, was missing. On the checklist “Monthly Test and Inspection Routines” dtd 05OCT2019 (M2-Firefighting Appliance, 7-fire extinguishers…) the check was marked “yes.” In the Maintenance Record Book, the Portable Extinguisher page has not been completed since July. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations… Life-saving appliances required by this chapter shall comply with the technical specifications of the LSA Code and National Regulations as appropriate. 6.1.6 Embarkation ladders 6.1.6.1 Handholds shall be provided to ensure a safe passage from the deck to the head of the ladder and vice versa. 6.1.6.2 The steps of the ladder shall be: .1 made of hardwood, free from knots or other irregularities, smoothly machined and free from sharp edges and splinters, or of suitable material of equivalent properties; .4 equally spaced not less than 300 mm or more than 380 mm apart and secured in such manner that they will remain horizontal. Vessel’s Port and Starboard embarkation ladders had broken rungs. Port embarkation ladder was switched with the pilot ladder because the condition of the pilot ladder was unsatisfactory for pilot usage - two broken rungs and one rung that was heavily dry rotted. Starboard embarkation ladder had temporary repairs to two rungs.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations… A vessel certified to operate in Restricted area II, III and Unrestricted shall be fitted with at least one 9 GHZ Marine Radar System for surface navigation approved by the Administration, with a radar screen mounted at the operating navigation station. The radar and its installation shall be suitable for the intended speed and operating area of the vessel and in operational condition at all times. Radar is not operational and Master claimed it has not been working since 10/20/2019. Master was unable to provide the reporting sheet for the Radar and PSCO’s witnessed the Master completing the sheet while onboard. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations… In ships of 50 m in length and over at least two of the lifebuoys provided with self-igniting lights in accordance with 4.12.2 shall also be provided with self-activating smoke signals. Each of these lifebuoys shall be capable of quick release from the place at which the ship is normally navigated where practicable or be of the throw over type. Lifebuoy located on the port side of the bridge does not deploy when the quick release pin is pull because the mounting bracket is parallel to the deck and will not deploy the lifebuoy as required. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations…The following guidelines… provides guidelines for conducting maintenance for the checklists for- Weekly, Monthly, Quarterly, Semiannual, and annually board. Per weekly tests requirements there should have been 19 weekly inspections conducted since the second week of June to the third week of October, the vessel could only provide records of 7 weekly inspections. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations… Every ship shall be provided with at least two anchors of sufficient weight one of which is provided with a chain cable of adequate strength and size and windlass, capstan or winch of suitable size for the cable and other anchor handling equipment to the satisfaction of the Administration. Vessel is still missing the Port Side anchor.
