ARC COMMITMENT

Each lifeboat shall be provided with an appliance which is capable of launching and recovery the lifeboat. Vessel free fall lifeboat launching appliance winch hydraulic coupling failed and the recovery of the lifeboat is inoperable. Vessel is required to have winch coupling repair and inspected for operation compliance by 4/18/25 winch follows DNV class report. Vessel is to provide class report of repairs to the USCG once completed.

The maintenance program of [46 CFR] 62.50-20(h) must include a check-off list to make sure that routine daily maintenance has been performed, fire and flooding hazards have been minimized, and plant status is suitable for unattended operation. Completion of this check-off list must be logged before leaving the plant unattended. During a review of the vessel’s Periodically Unattended Machinery Space (PUMS) Operations, identified the following: -The unattended check-off list was vessel specific and not part of the maintenance program. -The duty engineers were not logging completion of the check-off list prior to leaving the machinery plant unattended. -The duty engineers were standing a maximum of 8 hours of PUMS (at night), which impacted STCW Work/Rest hours. The 8-hour maximum unattended period was not reflected in the Unattended SMS procedure (VMS 6.03) or noted in any other reviewed procedure/standing order. Also, VMS 6.03 does not direct crew to log or complete the required check-off list. -Reviewed a recent duty engineer’s alarm and associated Watchkeeper entry. The engineer responded to an alarm at 0300. Prior to this, the pre-unattended round was completed at 2200. The timing of both the round and the alarm, caused an STCW Work/Rest non-conformity in Watchkeeper. When the engineer’s round was adjusted in Watchkeeper to reflect completion of an earlier round, the STCW Work/Rest non-conformity was cleared. Conduct an evaluation of PUMS, to include timing of the unattended round, the unattended check-off list, STCW Work/Rest hours (including Watchkeeper entries), and unattended procedures. Provide the results of this evaluation to USCG ACTEUR.

Periodic Safety tests must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. Systems addressed must include fire detection and extinguishing, flooding safety, propulsion, maneuvering, electric power generation and distribution, and emergency internal communications. Periodic Safety Test Procedures (PSTPs) must be developed and approved by USCG Marine Safety Center (MSC) or the Authorized Class Society (ACS). If ACS is utilized, ACS must certify to the OCMI the periodical safety tests have been approved on behalf of the U.S. Coast Guard and verified and satisfactorily demonstrated.

The OCMI will determine the specific manning levels for vessels required to have certificates of inspection by part B of subtitle II of title 46 U.S.C. The masters or individuals in command of all vessels, whether required to be inspected under 46 U.S.C. 3301 or not, are responsible for properly manning vessels in accordance with the applicable laws, regulations, and international conventions. The second assistant engineer (2AE) and one (1) of the oiler positions required by the COI minimum safe manning are currently vacant/unfilled. The vessel is to be manned in accordance with the COI minimum safe manning requirements prior to departure from its next port of call of Savannah, GA.

The masters or individuals in command of all vessels, whether required to be inspected under 46 U.S.C. 3301 or not, are responsible for properly manning vessels in accordance with the applicable laws, regulations, and international conventions. The vessel is currently sailing short 1 AB. Return to COI required crew compliment.

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. A lock was missing from the front cover of the switch box for the starboard steering gear, resulting in an opening to the electrical equipment within-blank or otherwise secure the opening. SOLAS II-1/45.1.3

Not less than one half of the total number of lifebuoys shall be provided with lifebuoys self-igniting lights complying with the requirements of paragraph 2.1.1 of the code. Lifebuoy on the main deck starboard aft has a self-igniting light that was inoperable. Make light operable. SOLAS III/7.1.3

No 3 fuel injector found to be faulty and caused RPM drop. Has been corrected and owner will conduct investigation and overhaul of all engine injectors. No Form B issued to vessel.
