Tankers shall be equipped with at least one portable instrument for measuring oxygen and one for measuring flammable vapour concentrations, together with sufficient set of spares. Suitbale means shall be provided for the calibration of such instruments. Crew is unable to calibrate testing equipoment due to expired or lack of span gas or board. SOLAS (14)II-2/4.5.7.1

Vessel is equipped with fixed gas sampling system for measuring hydrocarbons in double bottom spaces. However the crew isn't using this equipment. When PSCO's asked the crew to turn the system on it had every sample point cut out while loaded with cargo. Furthermore the crew is unfamiliar with how to enable sample points. SOLAS (14)I-11

In all cases the interval between any two such inspections shall not exceed 36 months. PSCO's noted that the vessel was dry docked 14FED2013 and the crew was unable to produce any underwater body inspection results since that date. SOLAS (14)I/10(a)(v)
