Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling, or daily service tank, having a capacity of 500L and above, situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occuring in the space in which such tanks are situated. PSCO observed that fuel shutoff valve for the emergency generator was unable to be actuated outside the space. (74 SOLAS (2004) II-2/4.2.2.3.4)

Resolution: Vessel repaired valve with spares and class surveyor attended to verify. Following, PSCO attended vessel and verified valve operation from outside the machinery space to be satisfactory.
To discharge ballast into the waters of the United States, the master, owner, operator, agent, or person in charge of a vessel subject to 151.2025 of this subpart must either ensure that the ballast water meets the ballast water discharge standard in 151.2030(a) use an AMS as described in 151.2025(a)(3) or ballast with water from a U.S. public water system, according to the schedule in paragraph (b) of this section.
Subject vessel, of keel laid date 26MAR2007 and ballast water capacity of 12116.6 m3 is past its compliance date per the implementation schedule 33CFR151.2035(b). Vessels current ballast water management method is exchange and discharge which is not authorized. Vessel has no BWTS installed. Vessel is not authorized to discharge untreated ballast into U.S. waters. This deficiency will not be cleared until CG examiners receive documentation that the vessel has installed a BWTS, has decided to use public water from the U.S. public water system or has updated its ballast water management plan to state that the vessels ballast water management method in U.S. waters is no discharge. 33CFR151.2035 15C

voyage, all lifesaving appliances shall be in working order and ready for immediate use.
Crew was unable to lower port side lifeboat due to inoperable davit launching devices. Additionally, the painter on the starboard side life raft was secured to the frame vice the weak link.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company (10.1). And meeting these requirements, the company should ensure that (10.2): inspections are held at appropriate intervals (10.2.1); any non-conformity is reported, with its possible cause, if known (10.2.2); appropriate corrective action is taken (10.2.3); and records of these activities are maintained (10.2.4).
As per ship’s procedures, the life boat is to be moved from its stowed position weekly.
As per ship’s procedures, the life rafts, to include the connection of the linkage of the painter and weak link, are to be inspected monthly.
According to work order SEA-1912-V002112, the port lifeboat was inspected without any discrepancies on 06 Dec 2019. However, on 11 Dec 2019 crew was unable to lower port side life boat due to inoperable davit launching appliance.
According to work order J502001 conducted on 06 Dec 2019, the connection of the painter was checked and found satisfactory. However, on 11 Dec 2019 the painter in the starboard side life raft was secured to the frame vice the weak link.
An internal audit focusing on the maintenance and inspection of the vessel’s lifesaving appliances is required within 3 months.
