Issued 8 March 2017 Resolved
Fuel oil for combustion purposes delivered to and used onboard ships wo which this annex applies shall meet the following. Not jeopardizes the safety of ships or adversly affects teh performance of machinery. Excessive water found in D.O. Service tank, which caused a blackout while pierside. After draining approximatley 8 inches of water still in service tank which can cause machinery failure.
Action required: 40 - Rectify deficiencies prior to next US port after sailing foreign
Resolved 5 April 2017
Resolution: 5APR17: Survey report states: regular maintenance of drain pipe in mist box was added in Planned Maintenance System (PMS) accordingly for preventing the re-currence.
