All arrangements used for pilot transfer shall efficiently fulfil their purpose of enabling pilots to embark and disembark safely. The appliances shall be kept clean, properly maintained and stowed and shall be regularly inspected to ensure that they are safe to use. PSCO observed cracks on at least five steps on the vessel’s port side pilot ladder. 74 SOLAS (2020) V/23.2.1

Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. PSCO observed that the vessel’s paint locker had buckets placed under the sprinkler heads in a manner that would have prevented the sprinklers from working as designed. These buckets had been inadvertently left there when the crew tested the system on 26 MAY 24. 74 SOLAS (2020) II-2 14.1.1
