Each door, hatch, or scuttle, used as a means of escape, must be capable of being opened by one person, from either side, in both light and dark conditions. The method of opening a means of escape must be obvious, rapid, and of adequate strength. Handles and securing devices must be permanently installed and not capable of being easily removed. A door, hatch or scuttle must open towards the expected direction of escape from the space served. The aft sliding door internal door handle is missing rendering door unable to be properly operated from inside cabin space. Replace/repair aft sliding door and ensure proper operation of door to the satisfaction of attending marine inspector. 46 CFR 177.500 (h)

Each item of lifesaving equipment with an expiration date must be replaced during the annual inspection and repair if the expiration date has passed. EPIRB Hydrostatic release unit (HRU) expired. Replace HRU with new or serviced unit to the satisfaction of the attending marine inspector. 46 CFR 185.126 (c)

(b)(4) Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Electric solenoid valves must not be used, unless used in addition to the manual valve. Shutoff valve did not fully rotate to the closed position. Vessel rep repaired prior to MI departure.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2 1/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel only had two 2.5 gallon fire buckets. Vessel rep placed third fire bucket on vessel prior to MI departure.

(d) A power-driven vessel of less than 12 meters in length may, in lieu of the lights prescribed in paragraph (a) of this Rule, exhibit an all-round white light and sidelights All around masthead light was found extinguished. Vessel rep replaced in kind prior to MI departure.

(d)(3) Each floating waterlight installed after March 11, 1997, on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. The clip must have a strength of at least 22.7 kilograms (50 pounds), and allow the waterlight to be quickly disconnected from the ring life buoy. Float missing corrosion resistant clip. Vessel rep installed new corrosion resistant clip prior to MI departure.

(b)(1)(ii) Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Educated vessel rep on importance of proper inspections and documentation of portable fire extinguishers.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Discovered three cracked longitudinal stitch welds along the keel starting approximately 2' aft of the main cabin fwd door. Complete repairs to satisfaction of the attending CG Marine Inspector. 46 CFR 176.600

The construction, installation, and operation of propulsion and auxiliary machinery, piping and pressure systems, steering apparatus, and associated safety systems. Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. The starboard jet bucket control cable boot seal has deteriorated away and is partially missing. Properly repair/replace starboard bucket control damaged boot seal. 46 CFR 182.100

Vessel did not have crew available to conduct crew drills. Conduct crew drills with marine inspector prior to carrying passengers.

Life jackets were stowed in the overhead storage and the location was not labeled. Mark location with number and size of life jackets.

During annual ISE found 25 plus cracked welds in the area from anchor locker bulkhead under fwd steps to just behide the first cross frame aft, along with one broken/missing long. Frame and 2 cracked frames in need of repair

Provide updated coast pilot, light list, and paper chart.

Properly repair the port MDE raw water cooling system. Raw water cooling hose ruptured causing the engine to overheat.

Properly repair / replace the ruptured stbd MDE raw water cooling hose, this hose supplies cooling water to the jacket water heat exchanger. Repair to the satisfaction of the attending CG Marine Inspector.

Crew was not available to conduct drills in this boat.

Vessel's STBD trib tab thru hull was leaking approximately 1 cup of water per minute into ER bilge. Make thru hull water tight.

Life raft sea painter was not connected to weak link.
