Accommodation and service spaces and control stations of cargo ships shall be by a fixed fire detedction and fire alarm system and/or an automatic sprinkler, fire detection and fire alarm system as follows, depending on a protection method adopted in accourdance with regulation 9.2.3.1. PSCO observed 2 faults in fire detection panel Zone 5, MCP 48, Upper deck Passageway Port-MCP. Vessel's electrician was unable to identify the fault. SOLAS 74 (2020), II-2/7.5.5, Restricted, Code 17.

The Company should establish procedures, plans, and instructions, including checklists as appropriate, for key shiipboard operations concerning the safety of the personnel, ship and protection of the environment. The various tasks should be defined and assigned to qualified personnel. Vessel's SMS procedure 04 Part 4 cargo / Dangerous Goods section 2.0 Description of progress, addresses procedures to be followed before comencing cargo operations and for carriage of dangerous goods. PSCO observed vessel currently loading Ammonium Nitrate UN 2067 without being issued a proper shipper's declaration, or a shipper provided certificate signed by the shipper stating that all the relevent conditions of the cargo required by the IMSBC Code have been met. The SMS documentation states the proper paperwork shall be recieved before cargo operations. Request external audit of vessel's SMS addressing issues regarding cargo operations within 90 days. SOLAS 74 (2020), IX/3.1 / ISM Code (2018), Regulation 7.

The carriage of solid bulk cagoes other than grain shall be in compliance with the relevent provisions of the IMSBC Code.IMSBC schedule for Ammonium Nitrate (UN 2067) states Pressure on the fire mains shall be maintained for fire-fighting and fire hoses shall be laid out or be in position and ready for immediate use during and discharging of this cargo. PSCO observed no fire hoses were laid out or charged during cargo operations. SOLAS 74 (2020), IMSBC Code Appendix 1, Deficiency Rectified, Code 10.
