An approved fixed gas warning system capable of monitoring flammable vapours shall be provided in cargo pump rooms, pipe ducts and cofferdams. Vessels fixed gas system could not continuously monitor for flammable vapours, and continuously gave a "fault" alarm. Vessels screen for fixed gas monitoring in the pump room was completely malfunctioning, showing inaccurate numbers and symbols spontaneously. 74SOLAS CONS. (2004) II-2/4.5.4.2 34ac
